As a Care Co-ordinator, you will play a key role in supporting patients to access the right care at the right time. Main duties of the job

The Care Co-ordinator will support the delivery of high-quality, person-centred care by coordinating services for patients with complex or long-term health needs. Working closely with GPs, nurses, the wider multidisciplinary team, community services, and external agencies, the post holder will help ensure patients receive the right care at the right time.

Key responsibilities include coordinating patient care, monitoring referrals and care pathways, supporting multidisciplinary team meetings, maintaining accurate patient records, communicating effectively with patients and carers, and signposting patients to appropriate health, social care, and voluntary sector services. The Care Co-ordinator will also support proactive patient management, contribute to service improvement, and ensure all work is carried out in line with practice policies, NHS standards, information governance, and safeguarding requirements.

The Care Co-ordinator plays an important role in supporting the delivery of high-quality, patient-centred care. Working as part of the multidisciplinary team, the post holder will coordinate care for patients with complex, long-term or multiple health needs, ensuring they receive timely, appropriate, and well-coordinated support.

The role involves working closely with GPs, nurses, practice staff, community services, and external organisations to improve communication, reduce barriers to care, and enhance the patient experience. The Care Co-ordinator will provide administrative and coordination support while helping patients navigate health and care services.

Person Specification

Experience Essential

Experience Essential Previous experience working as a Care Co-ordinator within a GP practice, Primary Care Network (PCN), or other NHS healthcare setting. Experience coordinating care for patients with complex, long-term, or multiple health conditions. Experience working within a multidisciplinary team and liaising with healthcare professionals, community services, and external agencies. Experience managing patient caseloads, care pathways, referrals, and follow-up activities. Experience handling confidential information and maintaining accurate patient records. Experience using EMIS Web Experience managing a busy workload and working to competing priorities.

Desirable

Desirable Experience working within Primary Care Networks (PCNs). Knowledge of personalised care, care planning, and population health management. Experience supporting frailty, long-term condition management, or proactive care programmes. Experience contributing to quality improvement initiatives within primary care.
