About this role

Job summary

A well-respected village surgery is looking for an experienced community matron to join our team to work with our elderly and frail patients, ensuring positive and holistic clinical case management for patients with long term conditions and other complex medical needs.

In addition, the post holder will also work in conjunction with our in-house clinical team to provide the delivery of minor illness clinics and support the practice nursing team in the delivery of vaccination campaigns.

Orchard surgery is an established community-based village surgery with 8500 patients, 4 GP Partners, 3 salaried GPs, 2 practice nurse, and 3 health care assistants. We are a friendly, supportive team, who go above and beyond for our patients. The clinical team meet daily for morning huddle and coffee and monthly clinical meetings are supported by an experienced and friendly administrative team.

Main duties of the job

The post holder will be required to provide clinical case management to the elderly and frail population of the surgery, working both from the surgery and in patients own homes as appropriate, and leading ward rounds of patients in our care, residential and learning-disability homes as needed. You will be supported on ward rounds by a GP registrar. This will involve dealing with long term conditions and other complex medical and social problems, ensuring the responsible delivery of safe, effective and evidence-based clinical care. The primary function of the role is to maximise the patients health and wellbeing by providing continuity and to clinically assess and treat patients to reduce unnecessary admission to acute services. You will also contribute to the service delivery, clinical governance and quality improvement at the Surgery.

Person Specification

Qualifications Essential

Adult Nursing qualification ( degree level) NMC registered Enhanced DBS clearance Minimum 5 years post registration Experience of Multi Agency working Experience of initiating and implementing clinical care/protocols

Experience Essential

Experience of working with patients with long term conditions such as Diabetes, Asthma and COPD Ability to understand and manage a diverse workload Excellent interpersonal skills Independent prescriber Palliative / end of life care skills Clinical assessment skills

Desirable

Self management and motivational skills Awareness of the long term condition management agenda Ability to work under pressure and manage a diverse workload
