Make a difference to people's lives every day.We are looking for an enthusiastic and motivated Community Occupational Therapist to join our Community Health and Social Care Team on a 12-month fixed-term contract.This is an excellent opportunity for a qualified Occupational Therapist who is passionate about supporting adults to remain as independent as possible within their own homes and communities. Working as part of a multidisciplinary team, you will undertake strengths-based assessments, identify practical solutions to promote independence, and support individuals to achieve outcomes that matter to them. You will work with adults with a wide range of physical disabilities, health conditions and support needs, providing specialist Occupational Therapy assessment, advice, equipment recommendations, moving and handling solutions, and adaptation recommendations

Main duties of the job

About the RoleYou will:

Complete person-centred Occupational Therapy assessments. Promote independence through a strengths-based approach. Assess for and recommend equipment, minor and major adaptations. Undertake moving and handling assessments. Work collaboratively with colleagues across Health, Social Care, Housing and voluntary sector organisations. Contribute to safeguarding responsibilities and Care Act assessments where appropriate. Manage a varied caseload with support and supervision from experienced Occupational Therapy colleagues.About YouWe are looking for someone who:

Holds a Degree or Diploma in Occupational Therapy. Is registered with the Health and Care Professions Council (HCPC). Has excellent assessment, communication and problem-solving skills. Is committed to person-centred and strengths-based practice. Works effectively as part of a multidisciplinary team. Is able to prioritise workload and manage competing demands.What We Offer
