Do you have experience providing compassionate care and support to patients and families in a health, social care, community or hospice setting?
We are looking for a dedicated Health Care Assistant to join our Hospice at Home team, providing high-quality palliative and end-of-life care to patients wherever they call home. This is a rewarding opportunity to support people at an important time in their lives, helping them receive safe, personalised and compassionate care in their preferred place of care.
Main duties of the job
Working under the supervision of Staff Nurses and Specialist Practitioners, you will deliver person-centred care and support to patients, families and carers. This will include assisting with personal care, supporting symptom management, observing and reporting changes in condition, maintaining accurate records and communicating sensitively with those receiving care and the wider multidisciplinary team.

To work under the direction of a Registered Nurse both independently and as part of a multi-disciplinary team to deliver holistic, high quality care to palliative and end of life care patients, their carers and relatives in the Community and /or on our Inpatient Unit.
To participate in a seven day per week operational service at the moment we are recruiting for day shifts only.
Person Specification
Experience Essential
Experience of working in a multi disciplinary team. Experience of working in a social care setting. Experience of working with vulnerable people.
Qualifications Essential
Care Certificate, NVQ 3 (QCF level 3 or equivalent)
Desirable
Any other relevant care qualifications.
Skills Essential
Excellent interpersonal and communication skills. Effective time management and organisation skills. Proficient Microsoft Office skills.
