Job summary
The Health Care Assistant will be working under the direct supervision of the senior practice nurse and strictly in accordance with specific practice guidelines and protocols, the healthcare assistant will assist the practice clinical team in the provision and delivery of prescribed programmes of patient care across Marlow Medical Group.
The post-holder will work collaboratively with GPs, Advanced Clinical Practitioners, Clinical Pharmacists, Nursing Teams, Care Home Staff, Community Services and Secondary Care Providers to deliver high-quality, person-centred care to some of the most vulnerable and medically complex patients in the healthcare system.
The role combines long-term condition management, proactive care planning, and admission avoidance.

Main duties of the job
The post-holder will
NHS health checks
ECG recording
Phlebotomy
Chaperoning duties
Processing and management of laboratory samples requested by GPs/nurses
Vaccine/cold chain storage, monitoring and recording
Surgical equipment and vaccine re-stocking and stock rotation
Clearing and re-stocking consulting rooms
Preparing and maintaining environments and equipment before, during and after patient care interventions
Assisting in the assessment and surveillance of patients health and well-being
Undertaking specific clinical activities for named patients that have been delegated and taught specifically in relation to that individual
Helping to raise awareness of health and well-being and how it can be promoted
Assisting with the collection and collation of data on needs related to health and well-being
Manage residents with multimorbidity and complex care needs.
Undertake anticipatory care planning.
Recognise and manage clinical risk and deterioration.
Coordinate onward referral where required.
Provide telephone and digital clinical consultations.
Work within professional scope of practice and competence.

Person Specification
Qualifications Essential
Evidence of completion of Care Certificate. Level 3 Safeguarding Children and Adults. Basic literacy and numeracy skills. Enhanced DBS Clearance. Occupational Health Clearance.
Desirable
NVQ Level 2 or Level 3 in Health and Social Care (or equivalent). Basic Life Support, Moving & Handling, Phlebotomy, or other relevant clinical skills.
Technical Skills Essential
Effective verbal and written communication Ability to follow instructions and work under supervision Ability to recognise and report changes in patient condition Basic IT skills, good organisational skills
Desirable
Ability to undertake delegated clinical tasks following training experience using electronic patient records ability to prioritise workload independently confidence managing challenging situations
Experience Essential
Evidence of ongoing personal and professional development. Experience managing frailty and complex patients. Experience of providing care or support to patients and service users Experience working as part of a team Experience maintaining confidentiality.
