Job summary
We are looking to recruit an enthusiastic and dynamic Independent Prescribing Pharmacist to join our growing multi-disciplinary primary care team.
You will work alongside our multi-disciplinary clinical team of GPs, Nurse Practitioner, Pharmacists, Pharmacy Technician and Nurses to deliver high quality patient care.
This is a maternity cover vancancy and is therefore available on a fixed term or locum basis for up to 1 year. We are looking for a Pharmacist with prior primary care experience to work 2-3 days per week. Protected learning time, CPD, and support from a dedicated GP supervisor will be provided to help you flourish within the role.

We welcome applications from qualified Independent Prescribing Pharmacists from all backgrounds, but experience in a GP practice is preferred.
Main duties of the job
With support from your dedicated GP supervisor, you will provide care for our registered patients. You will demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills, for patients within the service. You will demonstrate critical thinking in the decision making process. You will work collaboratively with the healthcare team to meet the needs of the patients and support the delivery of policy and procedures. You will provide a holistic and clinical service, with support and supervision from GPs, implementing agreed management plans and following approved protocols as appropriate.
The right candidate will also have the opportunity to take leadership of chronic disease management within the practice, running dedicated clinics to help our patients to manage their condition.

JOB RESPONSIBILITIES
Deliver chronic disease clinics for patients within your scope. The practice currently runs specialist hypertension, diabetes and respiratory clinics for our patients. Offer minor ailment appointments to patients who have been triaged by a GP, if within scope. Deliver the practice medicines optimisation plan and deliver patient services as determined by practice policy and local and national guidance. Maximise cost-effective prescribing and improve the quality of patient care. Manage the practice repeat prescribing process, maximising efficiencies where appropriate. Review patient discharge summaries, updating patient medications where appropriate.

Experience Essential
2 years post-qualification experience working as a pharmacist in the UK
