About this role

Job summary

To carry out health and wellbeing assessments and interventions within a clinic setting, in the community or patients' homes as required.

To address the health and wellbeing needs of patients experiencing their First Episode of Psychosis or At-Risk Mental State.

To work in partnership with service users with complex mental health needs and their carers, as part of a multi professional care planning approach

To provide support, education, and monitoring for service users in relation to medications prescribed by the early Intervention team prescriber.

To assist in the development of the Nursing Team, offer supervision to Band 3 Health Care Assistants, work with other agencies to ensure the highest standard of patient care.

To champion health and wellbeing within the staff team.

Main duties of the job

Undertake a comprehensive physical health nursing assessment as detailed in guidance for service users experiencing SMI including First Episode Psychosis. This will include blood sampling, recording ECG's, monitoring prescribed medication, and following guidance for treatment with antipsychotic medication.

To support people to develop and achieve the outcomes of their Health Care plan, working with other agencies to ensure the highest standard of patient care

Provide a range of health and wellbeing information and support to service users and their families. This will include advice and information relating to healthy eating, exercise, weight management, smoking and substance misuse.

To liaise effectively with all members of the multi-disciplinary team and other agencies involved in the care of the individual thereby maintaining and enhancing close professional working relationships.

To work with individuals and their families at a variety of venues including patients own homes, hospital bases and a range of community settings.

To record and maintain all professional, personal and clinical information of patients in a confidential manner according to the current confidentiality Policy of the Trust and in line with current IT systems and Trust Policies.

Support junior staff who escalate concerns raised by patients in their clinics-such as Safeguarding/Medicine Management/Physical health concerns.

Undertake regular supervision and PDR in accordance with Trust policy.

Person Specification

Qualifications Essential

BSc/ MSc Degree in Mental Health Nursing

Desirable

Evidence of Post Registration Training in a relevant topic

Experience Essential

Working with people experiencing SM Supervision and training of learners/junior members of staff

Desirable

Experience of working in a clinic setting conducting physical health investigations.

Knowledge Essential

Knowledge of Care management, Multi-agency working, Mental Health Act and Mental Capacity Act, safeguarding principles. Knowledge of Early Intervention in Psychosis treatment approaches and evidence-based practice in the care of people with mental illness. Including medicines management and administration.

Skills/Abilities Essential

Strong interpersonal and organisational skills Able to self-start, lead and complete complex tasks Good written and verbal communication skills including ability to use electronic patient records

Desirable

The necessary skills to conduct a full physical health check including Venepuncture and ECG
