Job summary

Healthcare Assistant - Market Weighton Surgery - up to 37.5 hours per week

We are looking for a compassionate and enthusiastic Healthcare Assistant to join our friendly nursing team at Market Weighton Surgery. This is an exciting opportunity to work in a welcoming General Practice where you will play a vital role in supporting patients with their healthcare needs while developing your own skills and experience in a rewarding clinical environment.

Although advertised as a full time role, we will consider part time hours for the right candidate(s).

Main duties of the job

The Healthcare Assistant (HCA) will work as part of the multidisciplinary team at Market Weighton Surgery, supporting the nursing team and GPs in delivering safe, high-quality patient care. The post holder will undertake clinical and administrative duties within their scope of competency, working under the supervision of a registered healthcare professional and in accordance with YHP policies, clinical protocols and national guidelines. The role will contribute to improving patient health outcomes through health promotion, preventative care and the effective management of long-term conditions.

Examples of duties will include:

Undertake new patient health checks and health assessments,venepuncture/phlebotomy, recording and documenting clinical observations including blood pressure, pulse, temperature, oxygen saturation, height, weight, urinalysis, ECGs, blood glucose monitoring, provide support to nurses for chronic disease monitoring, wound care, vaccinations, assist in minor surgery, charperoning, promotion of healthy lifestyle.

Job Purpose

The Healthcare Assistant (HCA) will work as part of the multidisciplinary team at Market Weighton Surgery, supporting the nursing team and GPs in delivering safe, high-quality patient care. The role will contribute to improving patient health outcomes through health promotion, preventative care and the effective management of long-term conditions.

Key Responsibilities

Clinical Duties

The post holder will:

Undertake new patient health checks and health assessments. Perform venepuncture/phlebotomy safely and effectively. Record and document clinical observations including: Blood pressure Pulse rate Temperature Oxygen saturation Height, weight and BMI Urinalysis Blood glucose monitoring Carry out and record ECGs. Support practice nurses in chronic disease monitoring and recall programmes. Undertake wound care and dressings as delegated. Facilitate ambulatory blood pressure monitoring and provide patient guidance. Assist in minor surgery procedures and clinics. Act as a chaperone when required. Administer vaccinations and immunisations following appropriate training and competency assessment. Recognise deterioration or significant changes in patients' conditions and escalate concerns appropriately. Promote healthy lifestyle choices and support health promotion initiatives. Patient Care

Deliver safe, compassionate and person-centred care. Communicate effectively with patients, relatives and carers. Treat all patients with dignity, respect and compassion. Provide reassurance and support during clinical procedures. Maintain patient confidentiality at all times. Administrative and Information Management

Accurately document all interventions within the clinical record system. Ensure accurate coding and data entry where appropriate. Support audit activities and quality improvement initiatives. Maintain up-to-date knowledge of SystmOne and other clinical systems used within YHP. Comply with Data Protection legislation, Caldicott principles and Information Governance requirements. Stock and Equipment

Ensure clinical rooms are prepared, stocked and maintained. Monitor and replenish clinical supplies as required. Maintain refrigerators and equipment in accordance with policy. Ensure specimens are correctly handled and prepared for transportation. Support the safe disposal of clinical waste and sharps. Team Working

Work collaboratively within the multidisciplinary team. Contribute positively to service development and practice improvement. Support students, trainees and less experienced colleagues where appropriate. Demonstrate flexibility to meet service needs across YHP practices if required. Professional Responsibilities

Knowledge Essential

Qualifications Essential

NVQ Level 2/3 in Health & Social Care, Care Certificate or equivalent experience Trained in venepuncture/phlebotomy GCSE (or equivalent) English and Maths
