Job summary
We are looking for a caring and enthusiastic Healthcare Assistant - HCA to support the delivery of Annual Health Checks for patients with Severe Mental Illness - SMI.
Working under the supervision of a clinician, you will support patients by carrying out health checks, gathering information, and helping them access the care and support they need. All work must be carried out in strict accordance with organisational policies, procedures, guidelines, and clinical protocols.

Main duties of the job

Cleaning and de-sanitising and maintenance of equipmentSupplies, equipment re-stocking and stock rotationPreparing and maintaining environments and equipment before, during and after patient care interventions.Assisting in the assessment and surveillance of patients health and well-beingRaise awareness of health and well-being and how it can be promotedAssisting with the collection and collation of data related to health and well-being Completing administrative tasks in line with the role including contacting patients over the phone to complete health checks Any other duties deemed appropriate for the role by the responsible clinician
