Assesses the patient and prepares an individual Master Treatment Plan and monthly reviews and updates thereafter. Performs mini-mental status exam, psychosocial assessments, facilitates both individual and group therapy, documenting all interactions in a time frame compliant with company, state, and federal guidelines. Is responsible for contacting any patient who is absent for 3 or more days to determine reason for the absence and when they plan to return to the program.

• Must able to perform crisis evaluations and discharge planning • Performs a mini-mental status exam and psychosocial assessment on all new patients. • Performs other duties as assigned. • Provides group and individual therapy and prepares timely documentation in compliance with company, state, and federal guidelines. • Performs related duties as required

Required:

• Education: Master's degree in Social Work or related field. • Experience: • 2 years of related experience. • Work experience may not substitute for minimum education requirement.

• Licensure/Certification: • Currently licensed, certified, or registered to practice profession as required by law, regulation in state of practice or policy • CPR
