About this role
Job Summary:
Responsible for consultation and provision of dietetic services in Dialysis. Provides education, including orientation and monthly patient education, to patients, families, and staff. Develops, organizes, and implements programs to ensure the dietetic needs of the centers are met. Conducts performance improvement projects and participates in quality improvement activities.
Duties and Responsibilities:
• Demonstrates knowledge of clinical dietetics.
• Processes diet changes and new diets as received from inpatient areas (CDM backup).
• Rounds with providers:
a. Participates in the development of the patient plan of care.
b. Ensures nutritional assessments
c. Verifies diet orders
• Rounds with a patient with CDM. Provides patient education as needed.
• Attends IDT meetings each afternoon (CDM backup).
• Ensures documentation meets current standards and practices, including EMR documentation.
• Makes recommendations for needed changes in service delivery (sharing observations and input)..
• Other duties as assigned.
