As the Dementia Care Partner Program Coordinator, you will serve as the primary point of contact for individuals and families enrolling in our Dementia Care Partners Program. This role is responsible for conducting comprehensive screening and enrollment of participants, administering standardized screening tools, and building trust through clear and effective telephone communication. The ideal candidate will excel at tracking and managing referrals efficiently, performing accurate data entry and creating encounters in the electronic medical record system, and communicating with providers regarding program referrals. Additional responsibilities include providing administrative support and coverage for the Associate Director, occasionally covering health coach visits during staff absences, and completing various administrative tasks to support program operations.
Location: This is a remote position with occasional on-site requirements at Banner Sun Health Research Institute (10515 W. Santa Fe Dr., Sun City, AZ 85351) and attendance at an annual full-department retreat held at various locations throughout the Phoenix metropolitan area.
Schedule: M-F, 8am-4:30pm.

POSITION SUMMARY This position provides a variety of program support functions which contribute to the overall management of clients’ healthcare, quality of life, and efficient use of resources. Additionally, this position provides a higher focus on administrative duties to ensure program requirement compliance. This position is responsible for communicating with eligible participants. Responsible for on-going client data collection and documentation in the electronic medical record, as well as communicating such data to the team to meet regulatory requirements. Responsible for completing chart reviews to determine program eligibility requirements to determine eligible cases for the program. This position is responsible for verbal communication with eligible participants and obtainment of data for program participation. CORE FUNCTIONS 1. Maintains basic knowledge of clinical techniques, software applications, or skilled trade. Maintains current knowledge of applicable policies and procedures. Reviews and analyzes data. Performs tasks required by program protocol, which may include obtaining patient medical history, systems review, and cognitive assessments on participants. Maintains quality assurance on designated protocols. Collects data as required. 3. Ensures uniform implementation of protocols by completing required documentation in a consistent, accurate and timely manner. Completes work with direct supervision. 4. May conduct visits with program participants. Provides educational training and information to patients and caregivers. May serve as a primary and direct contact person for patients and families. 5. Actively participates in program evaluation and process improvement processes. 6. Serves as a liaison to other departments, program partners as well as community agencies. 7. May be involved in the training or onboarding of other program staff. 8. Exhibits excellent interpersonal skills and a high level of customer service approach in all contacts with patients, caregivers, referral sources and staff. Responds to standard requests from internal customers. Interacts primarily with department personnel, cross-department personnel, internal customers, patients, caregivers and physicians. MINIMUM QUALIFICATIONS High school diploma/GED or equivalent working knowledge and/or may be in training for a technical/trades career in the healthcare field. Current and active Certified Nursing Assistant (CNA), Community Health Worker Certification (CHW), or Medical Assistant Certification in state of practice may be required in certain facilities and/or departments in accordance with regulatory requirements or facility/department policy. Moderate experience, typically gained through 1 year of relevant healthcare experience. Must have clinical knowledge in healthcare and working knowledge of medical terminology. Must be proficient with clinical care processes, clinical technology as well as use of the Electronic Medical Record. Must have highly developed critical thinking skills with the ability to prioritize and work autonomously as needed. Must be able to adequately convey messages and thoughts to a diverse population in both written and verbal formats. Must be able to maintain excellent organizational skills under the stress of deadlines. Must be computer literate and have proficient typing skills. Must possess the desire to work with the older population. PREFERRED QUALIFICATIONS Motivational Interviewing skills, previous healthcare experience or experience in dementia care are preferred. Multilingual a plus. Additional related education and/or experience preferred.
