Your Opportunity
This is a permanent full-time opportunity, working Monday to Friday from 9:00am to 5:00pm and based on-site at Castle Hill Village.
As a Registered Nurse, you will deliver clinical care and nursing support to clients living within our Seniors Communities. You will conduct clinical assessments, monitor client health, contribute to care planning and provide nursing interventions that support clients to remain safe and well in their chosen environment.
You will play a key role in ensuring clinical assessments, care plans and clinical documentation are current, comprehensive and regularly reviewed to support ongoing clinical compliance, best practice and quality resident outcomes. You will also monitor high-risk residents, coordinate follow-up care following hospital discharge, and respond proactively to changes in residents' health and wellbeing.
Working autonomously and as part of a multidisciplinary team, you will ensure clinical needs are identified, documented and actioned in line with aged care standards, regulatory requirements and Anglicare's commitment to person-centred care.
With compassion, professionalism and clinical expertise, you will:
Clinical Care & Assessment
• Deliver routine and complex nursing care, including wound care, injections and clinical monitoring
• Conduct comprehensive health and clinical assessments and identify changes in resident condition
• Case manages residents' clinical care to ensure ongoing health, wellbeing and quality outcomes
• Monitor residents identified as high risk and coordinate appropriate interventions
• Review residents following hospital discharge and coordinate ongoing care requirements
• Escalate clinical concerns appropriately and liaise with GPs, specialists and allied health providers
• Promote positive health outcomes through early intervention and preventative care practices
Care Planning & Clinical Documentation
• Develop, review and maintain comprehensive assessments, care plans and clinical documentation
• Ensure care plans reflect residents' physical, emotional, social and spiritual needs
• Complete documentation accurately and within required timeframes
• Ensure compliance with organisational policies, clinical guidelines and legislative requirements
• Support ongoing clinical governance and quality assurance requirements
Resident & Family Engagement
• Build trusted relationships with residents, families and representatives
• Support residents and families through case conferences and clinical discussions
• Provide clear communication regarding resident health, care planning and clinical outcomes
• Ensure residents are treated with empathy, dignity and respect in all interactions
Teamwork & Knowledge Sharing
• Share clinical knowledge and expertise with colleagues and care teams
• Foster collaborative relationships with clients, families and healthcare professionals
• Support a culture of continuous improvement, learning and professional development
Safety & Wellbeing
• Promote and maintain safe clinical practice
• Participate in workplace health and safety activities
• Report incidents, hazards and risks in accordance with organisational requirements
What You'll Bring
You are a compassionate and capable Registered Nurse who enjoys building meaningful relationships and making a genuine difference in the lives of older people.
You will also need:
• Current AHPRA Registration as a Registered Nurse is essential
• Minimum 3 years' nursing experience
• Experience in aged care, community nursing or a related healthcare setting (preferred)
• Strong clinical assessment and care planning skills
• Excellent written and verbal communication skills
• Ability to work autonomously and manage competing priorities
• Sound computer skills and confidence using clinical documentation systems
• Current Driver's Licence and reliable vehicle
