We are seeking an experienced and dedicated Advanced Social Worker to join our team, specialising in supporting disabled children and young people across the West of Cornwall. The successful candidate will demonstrate a thorough understanding of the unique challenges faced by disabled children and their families, working collaboratively with multi-agency partners to deliver person-centred, outcome-focused interventions. You will be expected to provide expert assessment, care planning, and safeguarding practice, ensuring the voice of the young person is central to all decision-making.
