What you'll focus on
Coordinating personalised patient care
You'll manage a caseload of patients, working closely with them, their families and carers to develop personalised care and support plans that reflect their individual needs, preferences and goals.
Supporting patients to navigate health and care services
You'll act as a central point of contact, helping patients access the most appropriate services, appointments and community support while ensuring they receive a seamless, joined-up experience across health and social care.
Empowering people to manage their own health
You'll provide information, guidance and practical support that enables patients to better understand their conditions, access self-management programmes and make informed decisions about their care.
Working collaboratively across multidisciplinary teams
You'll work alongside GPs, nurses, social prescribing link workers, health and wellbeing coaches and wider healthcare professionals to ensure patients receive coordinated, person-centred care.
Supporting multidisciplinary working and service delivery
You'll help coordinate multidisciplinary team meetings, support palliative care and safeguarding processes, contribute to Quality and Outcomes Framework (QOF) activities and assist with practice initiatives that improve patient outcomes.
Building strong relationships with patients and carers
You'll provide compassionate, responsive support, ensuring patients and those who care for them feel informed, listened to and supported throughout their healthcare journey.
Contributing to continuous improvement
You'll support the ongoing development of primary care services by identifying opportunities to improve patient pathways, strengthen collaboration and enhance the overall patient experience.

Experience working in health, social care or another patient-facing support role. Excellent communication and interpersonal skills.
