Description: MAPPS Youth Specialist
Job Summary:
• Plans and conducts weekly group patient education sessions and individual education sessions per month with each participant.
• Maintains a caseload of required number of participants.
• Makes appropriate referrals to other organizations and agencies.
• Conducts home visits in order to complete assessments and case plans.
• Plans and provides cultural, educational and recreational activities for program participants.
• Serves as a role model for participants and parents, by acting in a manner consistent with the program's and district's philosophy.
• Completes travel reports and weekly documentation.
• Documents services provided according to Medicaid specifications.
Qualifications:
• Requires a Bachelor's Degree in Social Services or related field and training.
• Experience and skills for efficient functioning of the position.
• Equivalent combination of education, training, and experience that provides the required knowledge, skills and abilities.
Working Conditions: Full time position. This candidate will report to an assigned supervisor. This position will be reviewed at the end of year.
