About this role

Primary Purpose:

• The purpose of this role is to provide targeted interventions, coordinate internal and external support services, and develop Individualized Support Plans (ISPs) to address non-academic barriers to student success. By connecting students and families to meaningful resources, the Wraparound Specialist helps the campus take a proactive, data-driven approach to improving outcomes, increasing family engagement, and reducing disciplinary incidents, chronic absenteeism, and disengagement.

Qualifications: Education/Certification:

• Master's degree from an accredited university

• Licensed Professional Counselor (LPC) or Licensed Master Social Worker (LMSW) or Licensed Chemical Dependency Counselor (LCDC)

Special Knowledge/Skills:

• Working knowledge of substance use disorders.

• Strong communication, public relations, and interpersonal skills.

• Ability to coordinate individual and group sessions.

• General knowledge of computer systems and software applications.

• Knowledge of trauma-informed care, restorative practices, behavior intervention strategies, and case management

• Reliable transportation to/from work and between worksites.

Experience:

• Experience in working with at-risk youth, families, and community agencies

• Experience with crisis intervention and community resource coordination preferred.

Major Responsibilities and Duties: Student Intervention and Support

• Identify students experiencing academic, behavioral, attendance, social-emotional, or family-related challenges.

• Conduct needs assessments to determine barriers to student success.

• Develop individualized wraparound support plans that align interventions with student needs.

• Provide ongoing case management and monitor student progress toward intervention goals.

Coordination of Services:

• Connect students and families with school-based and community-based services such as:

• Mental health counseling

• Substance abuse programs

• Housing and food resources

• Medical and wellness support

• Mentoring and family support agencies

• Coordinate referrals and ensure continuity of care between service providers and the school.

Family Engagement:

• Build trusting relationships with families to increase collaboration and support.

• Serve as a liaison between the school and home to improve communication and access to resources.

• Empower families by helping them navigate community agencies and support systems.

Campus Collaboration:

• Work closely with administrators, counselors, teachers, attendance staff, and behavior support teams to identify student concerns and implement interventions.

• Participate in student support meetings, attendance review committees, and behavior intervention planning.

• Provide staff with guidance on trauma-informed strategies and available student resources.

Crisis Response:

• Assist with crisis intervention for students experiencing immediate emotional, behavioral, or environmental crises.

• Coordinate urgent support services for students and families in need.

Data Collection and Program Accountability:

• Maintain documentation of interventions, referrals, student progress, and outcomes.

• Track trends in attendance, discipline, and student engagement to measure program effectiveness.

• Provide reports demonstrating the impact of wraparound services on student success indicators.
