TITLE: Community Health Liaison (Part-Time) PAY RATE: Hourly Rate of $18.00 CONTRACTUAL DAYS: Maximum 25 hours per week/ 12-months REPORTS TO: Director of Special Education & Student Services Job Goal: To facilitate, coordinate, and support the successful implementation of the division's healthcare partnership with Tri-Area Community Health Center and associated health grant initiatives. This role serves as a bridge among schools, families, and healthcare providers to remove health-related barriers to learning, coordinate student access to care, and promote overall student and family well-being. Qualifications:
• High School Diploma or GED equivalency required; an Associate's or Bachelor's degree in human services, health, education, or a related field is preferred.
• Strong interpersonal and communication skills, with a demonstrated ability to build trusting relationships with diverse families and medical professionals.
• Excellent organizational and time-management skills, with the ability to manage schedules and paperwork efficiently.
• Comfortable utilizing basic technology, including virtual meeting platforms (e.g., Zoom, Microsoft Teams) and data entry programs.
• Ability to work independently and maintain extreme confidentiality regarding student and medical records.
Performance Duties: Partnership Coordination & Liaison
• Serve as the primary on-the-ground point of contact between school staff, families, and Tri-Area Community Health Center personnel.
• Facilitate clear, aligned communication between clinical partners and school administration regarding scheduling, program logistics, and student needs.
Service & Session Facilitation
• Manage the day-to-day logistics of the health program, including scheduling appointments and coordinating student availability with teachers.
• Oversee the physical or virtual coordination of care; ensure the telehealth space and equipment are properly prepared, secure, functional, and confidential for student sessions.
• Distribute, track, and collect necessary parental consent forms and intake paperwork.
Family & Community Outreach
• Actively engage with families to introduce available health services, answer logistical questions, and assist them in navigating the enrollment process.
• Help identify and mitigate barriers keeping families from utilizing services (e.g., lack of technology, language barriers, or difficulty completing paperwork).
Grant Support & Data Maintenance
• Assist in collecting and organizing non-identifying data and tracking participation metrics required for grant compliance and reporting.
• Maintain accurate, up-to-date, and strictly confidential records of program utilization and outreach efforts.
Resource Integration
• Work collaboratively with school counselors, nurses, and social workers to identify students who may benefit from Tri-Area services.
• Help build awareness of the program within the school community through school newsletters, flyers, or brief staff updates.
Professionalism & Compliance
• Maintain the highest level of ethical behavior, strictly adhering to both educational (FERPA) and healthcare (HIPAA) privacy and confidentiality standards.
• Perform other relevant health-partnership duties as assigned by the supervisor.
• Attends staff meetings, division professional learning workshops, and specialized training sessions as required.
• Discharges all responsibilities and fulfills all obligations as specified in the contract, policies, and guidelines of the Floyd County School Board.
