Duties to include; To visit clients living alone in their homes weekly or weekly depending on the client’s . Provide engagement for clients through home visits, Emphasis on companionship and wellness. Through a smart phone mobile application evaluate the wellbeing of the client; through the smart phone application be able to make referrals to appropriate services. Works well on a team or on own.
