We have an exciting opportunity for a Substance Misuse Support Worker to join our integrated substance misuse recovery team at HMP Wymott. HMP Wymott is a large, Category C prison in Leyland, accommodating approximately 1,200 adult males, and we have a varied and complex population. Practice Plus Group provides primary care, mental health and substance misuse services across the wings, clinic areas, inpatient facilities, and reception services.
Joining our Substance Misuse Recovery Team of 10, you will be the first point of contact for patients on the wings - empower those whom wish to make changes in their behaviour to promote health and well-being through recovery from drugs and alcohol. This is a crucial role within the healthcare team, making a real difference to patients, and delivering outstanding care.
Salary
This position is full time, youll receive an annual salary of between £25,557 - £29,723 per annum.
Hours Shifts are 8:30am - 4:30pm, Monday - Friday. How will we support you?

Main duties of the job
As our new Substance Misuse Support Worker, your responsibilities will vary. Some of your duties will include:
Be the first point of contact of Substance Misuse patients Carry out inductions for new patients, and follow up on incidents from the day before Pick up referrals to the service (average of 35-40 per month) Carry out duty work, including reception - seeing new patients and carrying out assessments Take part in social prescribing initiatives Delivering outstanding care to patients, seeking out, listening to, and acting on their feedback - ensuring that their care is personalised and tailored to the individual Provide information and advice on substance misuse, alcohol, smoking cession, blood borne viruses and safe sex practices If you are interested in career development, we have a wide range of opportunities for you to develop your skills and experience, which include both internal and external options for development and learning.
