About this role
Job summary Working with a skilled Community Rehab team, you'll support people in their own homes so they can live safely and independently. You'll join Occupational Therapists, Physiotherapists and Associate Practitioners who work closely together to deliver person-centred care. This role makes a real difference to people with physical or neurological conditions, helping them regain confidence and independence. You'll be supported with supervision from both disciplines and opportunities to build skills across Occupational Therapy and Physiotherapy. PREVIOUS APPLICANTS NEED NOT APPLY
Main duties of the job What you'll do:o Carry out holistic assessments in people's homeso Agree goals and deliver personalised rehabilitation planso Support mobility and functional daily activitieso Recommend and support use of adaptive equipmento Work with other services to ensure safe, effective care What we're looking for:o Experience in a similar health or social care roleo Ability to manage a caseload with a good level of autonomyo Strong communication and teamwork skillso A flexible approach and willingness to learn across disciplineso Ability to travel to support patients in their homes

Level 3 Qualification in relevant subject (Sports & Fitness/ Leisure Activity/Exercise Science/Health & Social Care)
Desirable
Foundation Degree in Health & Social Care
Experience and Knowledge Essential
Experience of multi-disciplinary working Previous Experience as a therapy assistant or health care assistant Experience of working with people with physical and / or mental health problems. Experience of working in a care environment, undertaking a full range of clinical competencies and maintains a competency portfolio.
Desirable
Experience of working within rehabilitation services Experience of working within the community Experience of working with adults with complex needs Experience of team working
