Job summary
To provide a high standard of care and practice at all times, taking direction from Community Service Team Leader and respite care co ordinator
To work as part of a multi disciplinary team respecting the skills, expertise and contributions of others in the provision of holistic care and support to babies, children, young people and their families accessing the service.
To treat each child and young person as an individual respecting their dignity and providing care that is sensitive to their needs.
To undertake clinical duties both within the community setting and also within the Hospice
Main duties of the job
Main Responsibilities
Care
To work under guidance and supervision ensuring the provision of high quality care and practice; in accordance with Derian House Children’s Hospice policies, standard operating procedures and guidelines for designated children or young people.
To accurately monitor and document the care for the children and young people and ensure that the care provided adheres to individual care plans. Ensure that the children and young people’s daily routines and individualised needs are respected at all times.
To be responsible for monitoring, reporting and recording any changes in the condition of the children or young people being cared for, including health, emotional and social care needs.
To be responsible for administering medicines and pharmaceutical products to children & young people following Derian House Children’s Hospice policy, standard operating procedures and NMC standards as directed and supervised by registered staff.
To be competent in Derian at Home’s defined core clinical competencies and individual care needs as a Clinical Support Worker within the community setting.
Ensure the respect, dignity and rights of children, young people and their families are upheld at all times.
Act at all times in accordance with relevant legal, ethical and professional legislation and frameworks.
To assist, as requested and appropriate with the clinical audits to enhance the delivery of care. Contribute positively to maintaining and implementing the required clinical standards.
To be responsible and adopt a positive approach in the process of preventing and controlling infections.
To maintain appropriate clinical records ensuring that they are stored securely and confidentially.
Agree regular objectives with supervisor and demonstrate progress.
Work in conjunction with the wider Derian team to provide holistic care to babies, children, young people and their families.
At all times work within professional boundaries
Quality Assurance
Contribute to the management of quality assurance issues reporting and recording incidents, responding to complaints and feedback in line with Derian House policy and standard operating procedures.
Participate in the formation of risk assessments for children and young people in your care, reporting and recording all incidents, accidents and near misses, actively participating in any corrective action required to minimise future risk.
Contribute to the safeguarding of children & young people ensuring practice complies with Derian House Children’s Hospices and identifying any areas of concern and reporting them in a timely way to the Social Worker or a senior member of the care management team.
Communication
Regularly attend team and other meetings as appropriate, contributing positively to discussions and debates and participating in shared decision making.
Ensure effective communication including the dissemination of information accurately and timely to support effective team working.
At all times act professionally representing the organisation and maintain positive and effective communication both internally and externally in representing the service offered by the hospice.
Produce clear, legible and consistent written documentation in accordance with Derian House Policy

Health and Safety
To be responsible and assist the registered staff in the process of preventing and controlling infections.
Ensure all equipment is utilised and stored safely and ready for use to support the needs of children & young people whilst at Derian House. This will include the timely reporting of any faulty equipment.
Take reasonable care to avoid injury to yourself or to others and to co operate with Derian House in meeting its statutory requirements.
To undertake mandatory training as required by Derian House.
To adhere to Derian House Lone Worker policy and procedures.

While end of life care is a core part of what we do, many of our children and young people are supported by Derian House for many years before this, and that time is used to provide fun, comfort and lasting memories for the whole family. Our team is small, skilled and deeply committed, and you’ll play a vital part in delivering exceptional care, predominately out in the community; under-taking respite visits, supporting symptom management and memory-making amongst many other tasks, but also working in-house in our hospice in Chorley.
It is key that you know if you apply for this role you will most of the time likely to be working autonomously, supported by a multi-disciplinary team to make decisions to best support families. As this is a role where you will be supporting families outside of the hospice, there will be many occasions where you are working alone, and need to be comfortable with this, with our systems and processes in place to make sure you are safe.
As our service grows and our 3–5 year plan takes shape we’re creating new opportunities for nurses who want to develop and thrive. We are investing more in community and rotational roles that work across our inhospice and community services, giving you the chance to build breadth of experience in a genuinely varied role.
