Job summary
An exciting opportunity has arisen for a compassionate and motivated Registered Nursing Associate (RNA) to join the Frailty and Community Outreach Team within the Primary Care Network.
Working as part of an integrated multidisciplinary team, the Nursing Associate will deliver high-quality, evidence-based nursing care to adults living with frailty, multiple long-term conditions and complex health needs across community and primary care settings.
Main duties of the job
The Nursing Associate will work as part of the PCN multidisciplinary team, providing person-centred care to adults with long-term conditions, frailty and complex health and social care needs.
The role will include completing delegated assessments and reviews, delivering planned clinical interventions, undertaking home visits and supporting patients to maintain their independence and manage their health. The postholder will carry out a range of clinical skills, including physiological observations, venepuncture, diabetic foot checks, wound care and point-of-care testing, in line with their training and competencies.
They will work closely with GPs, nurses, allied health professionals, community services and social care colleagues, contributing to care planning, multidisciplinary meetings and coordinated care. The role will also involve recognising deterioration or risk, escalating concerns appropriately and supporting health promotion, medicines optimisation and admission avoidance.

East & Central Brighton Primary Care Network brings together nine GP practices, supporting a population of around 75,000 patients.
We have developed a broad multidisciplinary Outreach Team spanning occupational therapy, physiotherapy, social prescribing, paramedicine, health coaching, nursing, mental health and pharmacy. Job description Job responsibilities Clinical Care
Provide compassionate, person-centred nursing care for adults with complex physical, mental health and social care needs.
Undertake delegated nursing assessments within scope of practice, contributing to the development, implementation and evaluation of individualised care plans.
Deliver planned nursing interventions in accordance with evidence-based practice and local clinical guidelines.
Recognise changes in patients' physical, psychological and cognitive condition and escalate concerns appropriately.
Support patients and families to maximise independence, self-management and wellbeing.
Maintain professional accountability in accordance with NMC standards.
Long-Term Condition Management Support proactive management of diabetes, hypertension, COPD, asthma, cardiovascular disease, chronic kidney disease and frailty.
Undertake delegated reviews within agreed competency frameworks.
Support health promotion, lifestyle advice and self-management education.
Complete diabetic foot assessments
Contribute to QOF and enhanced service delivery.
Frailty and Community Outreach Undertake planned home visits for housebound patients.
Support comprehensive frailty assessments alongside registered clinicians.
Recognise deterioration using NEWS2 where appropriate.
Support admission avoidance and coordinated care.
Support patients approaching end of life alongside the multidisciplinary team.

Assessment and Risk Identification Identify and escalate risks relating to deterioration, falls, pressure ulcers, malnutrition, dehydration, infection, safeguarding, self-neglect, mental health deterioration, carer strain and social isolation.
Care Coordination Work collaboratively with GPs, Practice Nurses, Community Nurses, Allied Health Professionals and Social Care.
Participate in MDT meetings.
Support referrals and continuity of care.
Medicines Support medicines optimisation within scope of practice.
Administer medicines where trained, competent and authorised under local policy.
Monitor therapeutic effects and adverse reactions.
Health Promotion Promote healthy lifestyles, vaccination, falls prevention, nutrition and self-management.
Professional Responsibilities Maintain NMC registration and CPD.
Participate in appraisal, supervision and reflective practice.
Support learners and new staff where appropriate.
Maintain confidentiality and information governance.
Clinical Systems Use EMIS and or SystmOne to document care accurately, code activity and contribute to audit and quality improvement.
Lone Working Manage a community caseload safely.
Follow lone worker policies.
Work autonomously within professional competence.
Person Specification

Full UK driving licence. Access to a suitable vehicle. Ability to travel across the PCN. Ability to work safely as a lone worker.
Qualifications Essential
Registered Nursing Associate with current NMC registration. Foundation Degree Nursing Associate or equivalent. Evidence of continuing professional development. Qualified in Phelobtomy
Experience Essential
Experience as a Registered Nursing Associate in primary care, community or integrated care. Experience supporting adults with frailty and multiple long-term conditions. Experience contributing to nursing assessments and care planning. Experience of multidisciplinary working and community visiting. Frailty, long-term condition management, safeguarding, infection prevention, pressure ulcer prevention, falls prevention, Mental Capacity Act, consent and professional accountability. Essential Skills Venepuncture, clinical observations, NEWS2, urinalysis, wound care, pressure area assessment, patient education, documentation, communication, time management and risk assessment.
Desirable
Experience in a Primary Care. QOF experience. EMIS and/or SystmOne. Quality improvement and supervision experience.
