About the role
We are lookingfor an enthusiastic,skilled and motivated Stroke Specialist Nurseto join our newly establishedEarly Supported Stroke Discharge (ESSD)team, delivering care acrossHigh Peak and North Dales.
This is a rare opportunity to be part of the development of a new service, working in line with the established and highly regarded ESSD model used across Derbyshire. You will play a key role in enabling people to leave hospitalearlier and recover safely and effectively in their own homes, deliveringhigh-quality,evidence-basedstroke rehabilitation.

Main duties of the job
To provide nursing expertise within the multi-disciplinary ESSD team. This includes acting as an expert practitioner, liaising, guiding and advising the multidisciplinary team and external agencies in the provision of optimum nursing care for stroke patients on the ESSD pathway
To facilitate person centred care, enabling people with stroke to manage their condition effectively and independently, preventing complications and improving quality of life.
To participate in the development of effective, responsive, evidence based and patient centred services.
To assist and or lead on specific projects in agreement with the Team Leader and Operational Lead for the Service.
To work in close collaboration with other teams providing stroke care, and with partners across health and social care in locality, to support the delivery of effective ESSD services.

Skills Essential
Experience of working with stroke patients Experience of working as part of a multi-disciplinary team Ability to assess, manage and provide nursing interventions to patients following stroke Experience of psychological impact of stroke and ability to undertake appropriate mood assessments Experience of working/liaising with and supporting families and carers
Qualifications Essential
Degree (or equivalent qualification) in Adult Nursing NMC registered Post graduate experience and training in Stroke Nursing Care
Knowledge Essential
Minimum 2 years post graduate experience Evidence of working in stroke rehabilitation
Desirable
Minimum 1 year working in a community setting
