We are looking for a dedicated and motivated Senior Rehabilitation Assistant to join our Responsive Services team in this exciting and rewarding new role.
As a Senior Rehabilitation Assistant, you will play a key role in supporting patients to achieve their rehabilitation goals and remain safely at home. Working under the guidance of qualified clinicians, you will carry out basic assessments, provide equipment, and deliver rehabilitation programmes designed to maximise independence and improve quality of life.
In addition to managing your own delegated caseload, you will provide day-to-day support to Rehabilitation Assistants, helping to coordinate and allocate work across the team.
You will work closely with a friendly and supportive multidisciplinary team, including Physiotherapists, Paramedics, Occupational Therapists, Nurses, Health Care assistants, Rehabilitation Assistants and Administrative Staff. Responsive Services is an admission avoidance and facilitated discharge service, providing short-term rehabilitation and support to people who have recently been discharged from hospital or who are at risk of hospital admission.

Due to the community-based nature of this role and the geographical area covered by the service, applicants must hold a full driving licence and have access to a vehicle suitable for business use. Unfortunately, public transport is not a viable option for carrying out this role.

The role is based in Responsive Services which comprises of the Intermediate Care Team, Virtual Ward, Urgent Community Response and Referral Hub.
The Senior Rehabilitation Assistant will be an integral part of the Responsive Services team, supporting the rehabilitation assistants and allocating work as well as having their own delegated caseload.
The post holder will complete basic assessments of patients in their home and provide equipment under the guidance of a qualified clinician. They will carry out rehabilitation programmes with patients to increase independence to enable patients to remain in their own home.
