Maybush Medical Centre is looking to recruit a Care Navigator to join our friendly practice team
We are a successful practice with the highest standards of care and almost 10,000 patients. We are actively seeking to increase our list size
Main duties of the job
We are looking for a dedicated and friendly Care Navigator who will fit into our high performing team.
The role will be working from Monday to Friday; one week of mornings from 8:00-13:15pm and one week of afternoons from 13:15-18:30pm.

We are located just on the outskirts of Wakefield city centre, and have ample free parking on site and good local transport links.
The practice operates from purpose-built premises. We are SystmOne site.
Maybush is an accredited training practice for student nurses, GP's and medical students and offers opportunities to progress and develop, in both clinical and non-clinical capacities.
Job description Job responsibilities
Receive, assist and direct patients in accessing the appropriate services in a courteous, efficient and effective way Undertake a variety of administrative duties to assist in the smooth running of the Practice Ensure an effective and efficient reception service is provided to patients and any other visitors to the Practice Issuing patient on-line access passwords Deal with all general enquiries, explain Practice procedures to patients Receive and make telephone calls as required. Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery. Make appointments and telephone consultations ensuring callers are directed to the appropriate healthcare professionals Enter requests for home visits onto SystmOne, ensuring careful recording of all relevant details Communicate test results appropriately being aware of confidentiality at all times Handle repeat prescription requests and ensure that they are ready for collection by the patient within 24 hours Advise patients of relevant charges for private (non NHS) services, accept payment and issue receipts for same. Undertake chaperone duties during clinical examinations as required Enter patient information on to the computer as required. Ensure the kitchen is kept clean and tidy and the dishes are cleaned and put away as required. Keep the reception area, notice-boards and leaflet dispensers tidy and free from obstructions and clutter Providing clerical assistance to Practice staff as required, including scanning, coding, filing and photocopying Ensure Practice is closed down safely at night to include closing of window and shutters
Person Specification
Qualifications Essential
Good level of English and Maths
Experience Essential
Experience of working in a primary healthcare setting Evidence of appropriate continuing professional development activity to maintain up to date knowledge Evidence of working autonomously and as part of a team

Experience of working with General Practice
Knowledge and Skills Essential
Understanding and knowledge of policy related to delivery of Primary Care including the PMS contract, Clinical Governance and Quality and Outcomes Framework Understanding the health needs of Primary Care Understanding of equal opportunity and diversity issues Well-developed IT skills Excellent interpersonal, verbal and written communication skills Time management and ability to prioritise workload
