Job summary
We're looking for a motivated and patient focused Clinical Pharmacist (Independent prescriber) to join our friendly forward thinking GP practice. Working as an independent prescriber, you will play a key role in delivering high quality medicines optimsation, managing both acute and long-term conditions, undertakign medication reviews, supporting safe prescribing, interpreting blood test results, and working collaboratively with our multi-diciplinary team to improve patient outcomes. Job description Job responsibilities
Main Duties and Responsibilities:
To develop, implement and maintain practice-based pharmacy services covering the core and additional areas as defined within the Lancashire and South Cumbria ICB Practice Pharmacist service specification.
To provide face-to-face and telephone reviews for patients across all disease states within clinical competence as agreed with the GP partners
To provide prescribing support and advice to both clinical and non-clinical staff within the surgery
To provide diagnosis, treatment, and support to registered patients.
To examine, assess, diagnose, and prescribe for patients to manage both acute and chronic medical conditions via face to face, video and telephone consultation.
Prescribing and titrating of all medication associated with patient care when managing/controlling a medical condition
Analysis and action of patient blood test results
To facilitate effective transfer of patientcare from external organisation to surgery through the appropriate processing of medication related communications e.g. hospital discharge letters, consultant clinic letters
To undertake reviews of individual patients medication and make alterations as needed to improve patient care
To make recommendations for improving patient care and safety, to GPs and other healthcare professionals involved in the patients care
To utilise prescribing rights on an independent basis in the course of carrying out regular duties and responsibilities and within the pharmacists competence
To aid in the evaluation, development and implementation of guidelines protocols and formularies within the surgery
To appropriately inform GPs and other healthcare professionals of unnecessary drug expenditure and where this can be reduced without compromising patient care
To contribute to the design and delivery of initiatives regarding the management of medicines within the practice. To be responsible for implementing these initiatives at practice level, monitoring changes made and ensuring that benefits are maintained in the future
To identify areas of clinical risk at practice level and make recommendations to support the introduction of new working practices that will optimise the quality of prescribing and make more efficient use of practice resources
To develop and maintain an effective network and working relationships with other members of the practice
To participate in the daily signing of patient prescription requests.
To participate in multidisciplinary clinical meetings within the practice
To maintain own clinical and professional competence and be responsible for own Continuous Professional Development
To participate clinical and medication audit in priority areas, such as QOF, NICE guidance, NSFs and other agreed target areas
To work in collaboration with individuals appointed in the ICB and other local practices on local priorities where this also will be of benefit to registered patients or has been agreed by the partners
