Job summary
The Steps to Health service in Sunderland provides an innovative and dynamic approach to individuals who are at risk or who have developed a long term condition and would benefit from exercise and rehabilitation to keep them well. Education complements a programme of rehabilitation to provide knowledge and support enabling patients to make healthy choices and self-care.
We would like to invite applications for a Exercise Practitioner to join our team where you will play a key role working with specialists supporting assessments of patients referred to the service and initiating a plan of care.
You will be directly involved in education and in the delivery of exercise classes which can be face to face or virtual.
You will deliver Cardiac and Pulmonary Rehabilitation classes alongside Physiotherapists and Registered Nurses and provide education to patients.
A key part of the role is promoting the benefits of physical activity and healthy lifestyle (e.g smoking cessation or nutrition) to patient/clients, through education, in one to one or group sessions, specific to their diagnosis
Main duties of the job
To deliver programmes of cardiac and pulmonary rehabilitation in collaboration with an Allied health
professional, in accordance with national guidelines
To deliver exercise programmes in a range of settings, such as group settings, gyms, virtual
programmes or directly in patients own homes.
To deliver a range of high-quality physical activity opportunities which are progressive, graded and
reflect the needs of those referred to the service.
To evaluate the progress of a patients under their care, through appropriate outcome measures
To carry out telephone assessments for patients, including medical background, to assess their
suitability for an exercise programme
Multi-disciplinary team working
Caseload management and participation in clinical audit
Participate in delivering educational talks to patients specific to their diagnosis
To actively promote the health-related benefits of physical activity to patients, and to promote healthy
lifestyle changes
Support patients to develop self-care strategies and signpost to relevant agencies/services, or follow-
up exercise classes

Qualifications Essential
Recognised GP referral accreditation course or equivalent (NOC/NVQ Level3) NOS/NVQ Level 2 Certificate in Teaching Exercise and Fitness (for example, RSA/OCR Gym Instructor or Exercise to Music) BACPR Level 4 Cardiac Specialist Exercise Instructor or equivalent experience Be on the register of Exercise Professionals as a Level 4 Advanced Instructor
Desirable
Exercise qualification in Pulmonary Rehabilitation
Experience Essential
Experience of direct delivery of physical activity to groups and individuals with varying needs and abilities Experience of working with individuals with low, moderate and high risk factors for exercise/rehabilitation. Experience of advising individuals in relation to lifestyle and wellness Knowledge of the principles of exercise programming and design
