Job summary
An exciting opportunity has arisen within the Tri-Links Medical Practice for an accomplished, highly motivated Practice Nurse to join our friendly and forward thinking clinical team, for around 20-22 hours per week.
We are a busy GP practice with over 7,100 patients and as a practice we believe in delivering the highest standards for our patients.
The ideal candidate will be registered with the NMC and have experience in all fields of the job description. Primary Care experience is essential.
Main duties of the job
Lead on infection controlImmunisation clinics for children and travel vaccinesManagement of chronic diseases (Diabetes, Asthma, Hypertension etc) and educating patientsProvision of cytology services and family planningGeneral nursing care; to include wound dressings, removal of sutures, ECG's, blood pressure checksMaintain clinical areas and equipment, maintaining stock controlkeep accurate, relevant records both electronically and writtenUse of EMIS web clinical system and DocmanParticipate in clinical call and recall systems for QOF reviewsTo act as a focal point of communication between patients, doctors, nurses and other members of the primary health care teamTo maintain a safe and effective level of practice. To act in a professional manner at all times and adhere to practice policies, protocols and guidelines.To work as an effective team playerTo provide continuing care to patients across the whole practice population.To provide health maintenance and health promotional services to the well, offering educational supportive setting for patients to explore their health problems.

We pride our self in being a training practice from medical students to GP registrars.
We have 2 sites, which are both in Tamworth and the majority of staff work across both sites.

To lead on infection prevention and control by conducting regular audits, ensuring the clinical team adheres to infection control policies, procedures, and best practice guidelines, and implementing improvements where required to maintain high standards of patient safety and quality of care.To undertake chronic disease management according to relevant qualifications (i.e. asthma, diabetes, hypertension, coronary heart disease, COPD), incorporating progress monitoring, response to treatment, reducing unnecessary hospital admissions and promoting the preventative aspects of nursing practiceAdminister injections including baby vaccinations and travel injections as per patient group direction. Educate parents to encourage full childhood immunisation for their children to ensure vaccination targets are met.To ensure all clinical domains for quality outcome framework are met by completing annual reviewsCervical cytology clinicsContraception adviceTo facilitate and participate in multidisciplinary education within the practice.To support students of any discipline undertaking a work placement at the practice.BP monitoringUrinalysisAssist other clinical members when necessary.Provide lifestyle advice to patients making any necessary referrals within the Practice or external servicesUndertake wound care within capabilities.Undertake ECGsTake part in our annual flu campaign and clinics (out of hours clinics will be required during flu seasonTo advise and advertise health campaigns within the practice with support from the practice admin team and reception.Undertake high vaginal swabs as requested by a doctor.Being part of our Extended Access rotaPRINICIPAL RESPONSIBILITIES
