Job summary

As a PCN ANP/ACP, you will be working across two practices (three sites) in Rugeley The Horsefair practice group and Sandylane surgery. Our teams are ambitious and committed to delivering excellent services to those within our care. We are recruiting an Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) from a variety of professional backgrounds, including nursing, pharmacy and paramedicine. Main duties of the job

You will be responsible for the assessment of health and clinical needs of our patients, undertaking consultations as an autonomous practitioner, using your advanced clinical practice skills, and own clinical judgement to diagnose, treat, or refer patients.

Carry out consultations with patients taking a comprehensive patient history; physical examinations and use of your expert knowledge and clinical judgement to identify potential diagnosis. Decide on and carry out treatment, including the prescribing of medicine or referring to primary or secondary care specialist services within local agreed pathways and protocols as appropriate, Provide clinical advice and mentorship to junior clinical staff.

About us

Horsefair Practice and Sandy Lane Surgery have a combined patient population of approximately 21,000 and are led by two GP Partners. The practices operate across four sites in the Rugeley area, with three Horsefair Practice sites located within a short distance of one another and one Sandy Lane Surgery site.

We have a large, multidisciplinary clinical team comprising GPs, Advanced Nurse Practitioners (ANPs), Urgent Care Practitioners (UCPs), Practice Nurses, Healthcare Assistants (HCAs), Phlebotomists, Physiotherapists, a Pharmacy Team, a Mental Health Practitioner and a Social Prescriber.

Both practices are accredited training practices and are committed to supporting the education and development of healthcare professionals. Clinicians are expected to contribute to the supervision and mentoring of students, and we actively encourage and support the ongoing professional development of all members of our team.

Job description Job responsibilities

Provide autonomous clinical assessment, diagnosis and management of patients presenting with acute, urgent and long-term conditions through telephone, online and face-to-face consultations. Work within your professional scope of practice and be accountable for all clinical decisions and actions. Assess, diagnose, treat and evaluate patients using evidence-based practice. Prescribe medication (where appropriately qualified) in accordance with national guidance, practice protocols and prescribing legislation. Request, interpret and act upon appropriate investigations. Refer patients to secondary care, community services or other healthcare professionals where clinically appropriate. Undertake medication reviews and support patients to optimise treatment and self-manage long-term conditions. Contribute to the diagnosis and ongoing management of long-term conditions and support achievement of Quality and Outcomes Framework (QOF) targets. Undertake home visits and care home ward rounds where required. Recognise and respond appropriately to medical emergencies. Identify, assess and escalate safeguarding concerns relating to children and vulnerable adults in accordance with local and national policies. Work collaboratively with GPs and the wider multidisciplinary team, including Practice Nurses, HCAs, Pharmacists, Physiotherapists, Mental Health Practitioners and Social Prescribers. Support students and other healthcare professionals in their learning and development as appropriate. Participate in clinical audit, significant event reviews, quality improvement initiatives and service development. Maintain accurate, contemporaneous and legally compliant clinical records using EMIS Web. Maintain professional registration and comply with the standards of the relevant regulatory body (e.g. NMC or HCPC). Maintain competence through continuing professional development and mandatory training.Participate in Primary Care Network (PCN) initiatives and service development activities as required.This job description is intended as a guide to the main responsibilities of the post and is not an exhaustive list of duties. Person Specification

Experience Essential

Experience working as an Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) within Primary Care, Urgent Care or Emergency Care Experience of autonomous clinical assessment and decision making. Experience managing acute and minor illness presentations. Experience managing patients with long-term conditions. Experience of multidisciplinary team working.

Desirable

Previous experience in General Practice. Experience using EMIS Web. Experience undertaking care home rounds. Experience of telephone and online triage.

Qualifications Essential

Current professional registration with the NMC, HCPC or other relevant professional body. Holds the recognised qualifications and credentials required to meet the NHS England Advanced Practice framework and be eligible for an Advanced Practice Digital Badge (where applicable to the role). Master's level qualification in Advanced Clinical Practice (or equivalent) Independent Prescribing qualification. Evidence of continuing professional development

Desirable

Master's degree in Advanced Clinical Practice. Teaching, mentorship or assessor qualification.
