About this role

Job summary

This exciting role will provide clinical pharmacy services to various sites mainly based in Bodmin and/or Camborne Redruth. This role will include, attendance at ward rounds, support to self-medication, education of members of the multi-disciplinary team and patient/carer counselling. You will be supporting the lead pharmacists in the community setting to review and provide clinical pharmaceutical input and care to direct admission patients and those transferred from an acute trust. You will have the opportunity to develop managerial skills and support the wider pharmacy team including part of a 7day service rotation.

We welcome applications from individuals who are motivated to optimise patient care and develop pharmacy services. You will be supported with both the NMP and the ACP pathway. We will consider previous experience and applicants from any sector. For those with limited experience but the right passion and drive we would consider a band 6 to 7 progressional post. The role is a full-time post but alternatives will be considered.

Main duties of the job

The pharmacist, with the support of the lead pharmacists, will provide highly specialist,advanced clinical and professional pharmaceutical services to patients admitted to theCommunity Assessment and Treatment Units (CATUs), Same Day Emergency Care (SDEC)Units and community medicine areas (including minor injuries units) under the care of CornwallPartnerships Foundation Trust to promote the optimised, safe and cost-effective use ofmedicines. You will be a core member of the Trust's urgent care team, providing professionalpharmaceutical governance, medicines safety, medicines information, medicines finance,education and training and medicines optimisation support to the team and the service'spatients. You will build relationships and work with internal stakeholders and external partnersto support the delivery of high-quality pharmaceutical care to all patients.

Person Specification

Education / Qualifications and Relevant Experience Essential

Current professional registration - GPhC Professional qualification - MSC in pharmacy or equivalent (BSc, BPharm) Evidence of Continuous Professional Development

Desirable

Post Graduate Clinical diploma (mental health or clinical) Have experience of mental health

Knowledge and abilities Desirable

Hospital pharmacy experience Have knowledge of clinical governance, clinical audit and risk management Have up to date knowledge of mental health therapeutics
