Chedington is Hobbs new state of the art site centre which is growing at an exciting speed. This is allowing us to build upon our already experienced multidisciplinary team so that we can offer exceptional rehabilitation to more clients. At Chedington we are really lucky to have the opportunity to think beyond the normal remit for therapy and can be more imaginative with our approach. We are able to use adaptive sports such as paddleboarding, cycling, archery, air rifle shooting, equine therapy, adaptive hyrox and crossfit on site and off site with a large therapeutic element to these activities. We can do activities such as gardening and bush craft to work on functional rehabilitation skills. Then we can take clients to the beach, into local towns and countryside to work on putting skills into practice. Chedington has a number of pieces of exciting Neurotechology, including the only UK Lokomat that is able to treat both paediatrics and adults.
Main duties of the job
The person who gets this role will have the exciting opportunity to be able to join the team at Hobbs Chedington at an early stage of development of our service and will be able to help us mould what we can offer at Chedington. We receive a wide range of referrals from different sectors such as medico-legal, NHS, therapist led and charity based and self referrals. Hobbs partners with the Rugby Football Union (RFU) Injured Players Foundations (IPF), Help for Heroes, Injured Jockey Fund and the Spinal Injury Association (SIA), to name a few. The Rehabilitation Assistant will be at Hobbs Chedington as part of an experienced multidisciplinary team,working with inpatient and outpatient clients who have complex neurological conditions including Stroke, Traumatic Brain Injuries, Spinal Injuries, and complex poly-trauma including amputees. The role will involve working with the whole Multi-disciplinary team but with particular focus on adaptive sports and functional activities such as gardening. The post-holder will need to be:

DUTIES AND RESPONSIBILITIES
COMMUNICATION:
Communicate in a manner that positively represents Hobbs Rehabilitation at all times
Communicate professionally and effectively with other team members and external stakeholders
Communicate professionally and effectively with patients and carers in person and over the telephone.
Communicate professionally and effectively in all written communications with patients, carers, Hobbs staff and external stakeholders, through email, letters, written reports and comments on social media accounts and company websites.
Ensure client confidentiality is maintained at all times, in line with Hobbs Rehabilitation policies and procedures
Recognize peoples needs for alternative methods of communication and respond accordingly.
Respect and respond to clients' individual, cultural, and religious needs.
Gain consent for treatment and information-sharing, considering barriers such as dementia and cognitive impairments, in compliance with the Mental Capacity Act.
Support clients with communication and cognitive impairments to communicate effectively, with appropriate support from senior colleagues.

Communicate effectively with the multi-disciplinary team, recognising and highlighting any problems or concerns to seniors in a timely manner.
Communicate with clients effectively in one to one sessions as well as leading group exercise sessions
CLINICAL:
Create and provide clinically effective, high-quality one-to-one and group intervention to patients with acquired physical and cognitive impairments, as directed by the therapy team.
Deliver high-quality care in a variety of clinical settings including inpatient rehabilitation units, outpatient clinics, patient homes, care homes, and community locations.
Evaluate patients progress using published outcome measures, as directed by the therapy team, reporting back findings.
Liaise regularly with the Hobbs MDT and wider care teams to ensure continued patient safety and clinical effectiveness.
Support the organisation and planning of the Chedington caseload in conjunction with the service lead and other clinical team members to meet the patients and service needs, readjusting plans as situations change or arise.
Maintain up-to-date and accurate clinical notes using site-specific software and local procedures, in line with professional standards.
Ensure safe and competent use of clinical equipment, providing guidance to staff and patients where clinically appropriate.

ADMINISTRATIVE:
Ensure up-to-date and complete clinical records are maintained, using Hobbs-specific procedures and software

Answer the telephone and respond to emails and chat messages within a timely manner
Ensure client confidentiality is maintained at all times, in line with Hobbs Rehabilitation policies and procedures.
Support the creation and implementation of patient and staff timetables in your clinical area

Manage own delegated clinical caseload with appropriate time management and prioritisation, with support from the therapy team.

Assist in the implementation of service objectives and participate in relevant projects.
Represent the department in working groups and team meetings as required.
EDUCATION / TRAINING / CPD:
