About this Program

The Grampians Youth Prevention and Recovery Care (YPARC) program is a collaborative service between Wellways and Grampians Health Services.

The service is a step-down and step-up sub-acute mental health service for those within the catchment areas of Grampians Health. YPARCs are in a community setting and provide an option for people who are becoming unwell, or who are in the early stages of recovery from an acute illness and need a short period of additional support to strengthen their gains from spending time in an inpatient setting. They are supported to consolidate their community transition and recovery treatment plans.

About the opportunity

As Casual Recovery Worker, you’ll provide support for individual clients developed according to their needs, with a significant focus on achieving meaningful health, social and economic outcomes for people experiencing mental health issues. Supporting the team with assisting young people in their own pathway to recovery.

The Key Deliverables of the role are:

• Engage participants, their families, and friends in the program where appropriate and develop professional and trusting working relationships

• Conduct regular face to face support visits to connect with the person and to monitor and offer support around key areas

• Encourage linkages into a range of activities to support achievement of goals, including all other Wellways services.

• Develop individually tailored recovery plans with each participant which incorporate specific individual goals focusing on skill and knowledge development, including physical health and wellbeing and housing outcomes.

• Provide direct practical support to participants to attain the skills required to meet the goals identified to ensure social inclusion, including group activities as appropriate.

• Regularly reviewing the ways in which services are provided to ensure the program is service aligned with individual needs.

You will also need:

• Certificate IV or Diploma in Mental Health, AOD, or a related community services field, and relevant experience or an equivalent combination of relevant qualifications and experience

• Current valid Driver’s License and the ability to undertake travel for the role

• Current Working with Children Check and NDIS Workers Screening Check

• Right to Work within Australia

• Satisfactory National Police Records Check

• NDIS Worker Orientation Module Certificate
