Physical Therapist (PT) Desert Springs Post Acute (Soon to Be Willow Springs Healthcare Center) Location: Riverside County, CA Position: Physical Therapist (PT) – Full-Time or Part-Time Pay Range: $55.00 – $62.00 per hour DOE Join Our Rehabilitation Team Desert Springs Post Acute, soon to be known as Willow Springs Healthcare Center, is seeking a compassionate and dedicated Physical Therapist (PT) to join our growing therapy department. We are committed to delivering exceptional skilled nursing and rehabilitation services that help residents regain function, improve mobility, and achieve their highest level of independence. Position Summary The Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and delivering evidence-based therapy interventions designed to improve mobility, strength, balance, endurance, and overall functional independence. Essential Duties & Responsibilities

• Evaluate residents and develop individualized treatment plans based on clinical findings and physician orders • Provide skilled physical therapy services to residents in accordance with established plans of care • Assess and address mobility limitations, balance deficits, strength impairments, pain management, and fall risk reduction • Establish realistic, measurable treatment goals and modify plans as needed • Supervise and collaborate with Physical Therapist Assistants (PTAs) and rehabilitation staff • Complete timely and accurate documentation in compliance with facility, state, and federal requirements • Communicate resident progress and recommendations with physicians, nursing staff, families, and the interdisciplinary team • Educate residents and caregivers on exercises, safety techniques, and discharge recommendations • Ensure compliance with all professional, ethical, and regulatory standards Qualifications

• Graduate of an accredited Physical Therapy program • Current California Physical Therapist license in good standing • Current CPR certification preferred • Skilled nursing facility experience preferred but not required • Strong clinical assessment and treatment planning skills • Excellent communication and interpersonal abilities • Ability to work effectively in a team-oriented healthcare environment • Commitment to providing exceptional resident care and outcomes Benefits Eligible employees may enjoy:
