Concord Hospital-Franklin Skilled and Restorative Care provides short term transitional care for patients, following an acute hospital stay prior to returning home. Our patient centered health care team has the opportunity to work in a critical access setting, providing both acute care and subacute skilled care to the patients of the Lakes Region community. If you are looking to join an interdisciplinary team of high functioning individuals within the nursing, physical therapy and occupational therapy departments, and are passionate about making a difference in the lives of patients and famili
