Cleveland Clinic London are looking to recruit a Neuro Rehabilitation Pathway Lead to join our team.

We are looking for a confident and collaborative operational leader to support the delivery and ongoing development of rehabilitation services across Cleveland Clinic London’s Neurosciences Institute and wider rehabilitation pathways. This is a non-clinical leadership role focused on coordinating, optimising and overseeing the rehabilitation service pathway, with a strong emphasis on patient flow, timely admissions, pathway performance and service efficiency.

Acting as a central point of coordination, you will work closely with consultants, nursing leadership, therapy teams, operational services and external partners to support complex rehabilitation planning, external referrals and smooth progression through the pathway. You will also liaise with acute services, external hospitals, insurers, case managers, medico-legal teams, commissioners and community rehabilitation providers to support appropriate placement and continuity of care.

Reporting to the Institute Manager, you will contribute to accreditation activity, operational governance, pathway development and strategic growth initiatives, helping us deliver a high-quality, internationally aligned rehabilitation service.

What will your duties include?

• Coordinate and oversee rehabilitation pathways across inpatient and outpatient services, supporting timely admissions, patient flow, transfers, discharges and efficient progression through the rehabilitation journey.

• Maintain oversight of rehabilitation caseload activity, referral pipelines, waiting lists, bed utilisation and capacity, escalating operational risks where they may impact admissions, discharge planning or pathway performance.

• Act as a key operational liaison for rehabilitation services, building effective relationships with internal teams, referring hospitals, insurers, commissioners, case managers, legal representatives and community rehabilitation providers.

• Support the coordination of complex rehabilitation cases, including pathway tracking, funding coordination, external referrals, family meetings and escalation of operational barriers affecting patient progression.

• Monitor rehabilitation service performance, activity data and key operational metrics, producing reports and identifying opportunities to improve efficiency, patient experience, outcomes and service delivery.

• Contribute to accreditation readiness, operational governance, pathway development, audit, quality improvement and strategic growth initiatives aligned with rehabilitation standards and best practice.

What we need from you?

• Extensive experience in rehabilitation, neurosciences, complex discharge, pathway coordination or healthcare operational management.

• Experience managing complex caseloads, patient flow, rehabilitation pathways, admissions, discharge planning or wider service coordination processes.

• Confident working with multidisciplinary teams and external agencies to support effective pathway coordination and continuity of care.

• Strong ability to analyse service activity, operational performance data and key metrics to identify risks, trends and opportunities for improvement.

• Experience supporting governance, audit, quality improvement or service development initiatives within a healthcare setting.

• Experience of accreditation, regulatory or quality assurance processes, or working with insurers, commissioners, case managers or medico-legal stakeholders, would be advantageous.
