Band 5 Registered Nurse Community Rapid Response Team
30 - 37.5 hours per week
Hours: 9:00am to 5:00pm, 7-day service including weekend working on a rota basis (approximately 1 in 4 weekends, depending on service needs)
We're looking for a compassionate and motivated Band 5 Registered Nurse to join our Community Rapid Response Team.
This is an exciting opportunity to join an innovative service delivering urgent, palliative, person-centred care to patients in their own homes. The team supports admission avoidance, facilitates rapid discharge from hospital, and delivers care through our Community Virtual Ward pilot, helping patients with complex needs receive the right care, in the right place, at the right time.

Working alongside an experienced multidisciplinary team, you'll provide high-quality nursing care while developing your skills in community nursing, palliative care, and urgent community response.
Main duties of the job
What you'll do
Deliver high-quality, person-centered nursing care in the community.
Undertake holistic assessments and develop, implement and review care plans.
Provide end-of-life care, symptom management and support for patients with long-term conditions.
Work closely with GPs, hospitals and the wider multidisciplinary team to coordinate care.
Support patients, carers and families with compassion and professionalism.
What we're looking for
Registered Nurse with current NMC registration.
Excellent clinical assessment, decision-making and communication skills.
Ability to work independently and as part of a multidisciplinary team.
Palliative care experience is desirable but not essential.
Full UK driving licence and access to a vehicle is essential.

JOB PURPOSE
To provide skilled nursing care working under the supervision of the Hospice at Home Clinical lead to patients in a variety of community settings. The post holder will undertake community nursing duties involving delivery of nursing services and holistic assessments of patients health needs using a person-centred approach.
This includes providing quality and seamless nursing care to housebound patients and being proactive in co-ordinating their care needs as appropriate, avoiding unnecessary admission to hospital or to the Hospice.
In addition, the post holder will be involved in supporting carers and loved ones pre and post bereavement.
