As a Social Prescriber, you will work closely with community organisations, helping people access local services, reduce isolation, improve their wellbeing and take greater control of their health. Every day is different, and every conversation has the potential to make a real difference.
You don't need to have worked in healthcare before we're looking for compassion, resilience, excellent communication skills and a genuine desire to help people live healthier, happier lives.
Hours:7.5 - 12 hours per week (flexible working pattern)

Duties
Social prescribers can refer patients to local services that support a patients social, emotional, or practical needs. They work with the patient to understand what could help and find the best support available in the local community ie;

Carers Champion offering support and adviceTo keep accurate computer records of all patient contactTo attend meetings as required and those appropriate to the postTo participate in such other duties and responsibilities as may from time to time be reasonably assigned in accordance with training received both internally and externally.To carry out any training necessary to perform this role on a regular basis, including any update training to ensure competencies maintained.
Training
Full training will be given if required, supported by external Social Prescribing training courses.
