About this role

Position Purpose The Nurse Practitioner, Team Lead provides non-urgent primary care assessments in person or by telemedicine to clients. The position promotes continuity and effective transitions for clients and families and addresses their ongoing needs. The role involves collaborative leadership, formal education sessions, and coaching to enhance the provision of quality care.

Responsibilities Service

• Provide direct clinical assessments, consultations and primary care within the RN extended class scope of practice for clients with substance use, mental health issues and gambling concerns.

• Determine the needs of clients for urgent or emergent care.

• Use telecommunications technologies to relate to clients at remote locations.

• Provide telemedicine assessments to clients requiring immediate assessments to determine need for emergency room visit or other care.

• Establish a caring and therapeutic relationship with patients; demonstrates trust, respect, and honesty

• Provide education and information to patients individually or in group sessions on a range of health topics such as the use of medications for managing cravings and withdrawal.

• Promote decisions/actions that are consistent with a culture that values client-centred care.

• Collaborate with members of the interprofessional team, regarding care planning and follow-up for individuals with high risk or complex health problems.

• Collaborate with community agencies and diagnostic service providers to support the timely provision of comprehensive service to clients with substance use and/or mental health issues.

• Use highly developed communication and conflict management skills to assist members of the interprofessional team with the understanding and resolution of ethical and/or difficult situations.

• Provide education, coaching and support to staff and students to enhance problem solving and decision-making, regarding primary care to clients.

• Support the dissemination of research, knowledge and innovative practices.

• Engage in the collection and analysis of data, report writing in relation to program indicators.

• Monitor the telemedicine services and identify issues that require management attention.

• Provide recommendations regarding service gaps and necessary changes to enhance client and staff safety and/or quality of patient care. Leadership

• Manage the day- to- day operations of the clinical programs and services, in particular the clinical team who are trained as nurses. This includes: overseeing the training, scheduling, supervision and delegation of tasks.

• Work in collaboration with the Executive Director and the Clinical Director, to support the implementation of strategic plans, objectives and programs and services.
