Program Summary: The Path Home – Pine Villa is a short- term transitional housing program for hospital patients who no longer require acute care but still require care and support in preparation for a long-term living arrangement. Pine Villa specializes in supporting older adults (55+) with complex physical health challenges, mental health challenges, addiction or cognitive impairment. The Path Home – PIne Villa, provides client centered and goal-oriented support services that enable clients to continue their recovery in the community as they wait for permanent housing.  The Path Home – Pine Villa is a unique partnership between LOFT Community Services, SPRINT Senior Care and Sunnybrook Life Sciences Centre. It is funded by the Ontario Ministry of Health and Long Term Care. This partnership reflects the transformation of health care in Ontario which brings the expertise and some of the resources of a hospital setting to a community based transitional housing program. Some of these resources include on site clinical practitioners including an Occupational Therapist, Physiotherapist, Nurse Practitioner and Advanced Practice Nurse. Job Summary: The Registered Nurse (RN) will be part of an interdisciplinary team, who is expected to ensure the safety and security of the resident environment, to practice in accordance with College of Nurses of Ontario professional standards, and the policies and procedures as defined by LOFT Community Services. The RN will hold a unique and highly independent role as the clinical lead in an innovative practice setting. The RN will lead a team of Registered Practical Nurses (RPNs) to ensure that applicable delegated or controlled acts are safely and consistently delivered by Personal Support Workers (PSWs). The RN will be responsible for working with individuals from diverse backgrounds, lead clinical supervision of clients in the program; and providing training and leadership around the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment. The RN is directly responsible for clinical assessment and monitoring of client needs, assigning and monitoring controlled act delegation activities between RPNs and PSWs, and providing training and supervision to PSWs in the performance of delegated and controlled acts.  What You Will Do: Conduct comprehensive clinical assessments and develop individualized nursing care plans to support high-quality, client-centered care. Provide clinical oversight, training, and monitoring of delegated nursing acts across multiple program locations, ensuring regulatory compliance, accurate documentation, and timely reporting. Lead and support nursing practice by monitoring RPN performance, providing clinical guidance, and delivering direct nursing care when required. Oversee medication management, including administration, physician order transcription, medication reconciliation, MAR maintenance, and adherence to pharmacy policies and professional standards.
