The Social Worker provides services as a member of a multidisciplinary team. The incumbent will work collaboratively with community groups, agencies, and other professionals within the community. Human Resources Exemption: No Education- Bachelor of Social Work (BSW)Knowledge and Abilities- Expert knowledge and advanced skill in a variety of treatment modalities- Committment to self evaluation, professional development and ongoing self care- Understanding social determinants of health for program development & delivery- Knowledge in local human service organizations, resources, referral procedures- Systems Theory/Family Systems Theory, strength-based model, crisis intervention- Ability to work with a variety of individuals presenting with complex issues- Knowledge in individual assessment and counselling skills- Ability to work cooperatively in an integrated interdisciplinary team- Knowledge in health promotion and prevention strategies- Committed to delivering on patient and family centred care- Ability to complete case management assessments, follow-up and treatment- Strong interpersonal skills- Demonstrates strong communication and organiza...For more information on this job, please visit the employer's website.
