JOB PURPOSEThe Social Worker works collaboratively with a multidisciplinary team and partners with many groups such as government organizations, non-profit organizations, the private sector, and a multitude of community groups and other community organizations. The Social Worker helps to identify and improve the social determinants of health that impact the overall wellness of individuals, families, and the community, such as income, education, social environment, community inclusion, culture, h
