Job summary
We have a fixed-term 12-month maternity post for our Social Prescribing and Care Coordinator team. The post is for 30 hours a week. The successful candidate will be joining a team of 2 other social prescribers and 8 other care coordinators.
The post holder will be an integral part of the general practice team, working alongside multidisciplinary teams and wider community groups.
Main duties of the job
The social prescriber will hold 2-3 clinics a week to offer support and signpost patients referred to them by the general practice team. Outside of clinic time, they will be supporting the Care Coordinator Team, with ad hoc patient support to the practices' most vulnerable patients. The post holder will take part in case studies and reports to monitor and learn from support offered.
The post holder will have a direct line manager and GP support where patient concerns can be discussed.

a. Assess how far a patients health and well-being needs can be met by services and other opportunities available in the community.
b. Co-produce a simple personalised care and support plan to address the patients health and well-being needs by introducing or reconnecting people to community groups and statutory services, including weight management support and signposting when appropriate and it matters to the person.
c. Manage and prioritise their own caseload in accordance with the health and well-being needs of their population
d. Work as part of a multi-disciplinary team in a patient facing role using expert knowledge within the SPLW areas and promote social prescribing and its role in self-management
e. Build relationships with key staff in GP practices within the local PCN, attending relevant meetings, becoming part of the wider network team, giving information and feedback on social prescribing
f. Work in partnership with all local agencies to raise awareness of social prescribing and how partnership working can reduce pressure on statutory services, improve health outcomes and enable a holistic approach to care
g. Work with your line manager to access regular clinical supervision, to enable you to deal effectively with the difficult issues that people present.
h. Work alongside the Care Coordinator team to support internal health campaigns
i. Provide support for the annual health check recall campaigns.
j. Learn internal practice systems relating to patients appointments.

Qualifications Essential
- GCSE Grade A-C in English Language and Maths - General Practice experience
Desirable
- Social Prescriber Link Worker Training Qualification - Training in motivational coaching and interviewing
Experience Essential
- Experience of working directly in a community development context - Experience of working alongside multidisciplinary teams.
Desirable
- Experience of running Social Prescribing Clinics - Experience of data collection and monitoring information.
Knowledge and Skills Essential
- Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities. - Knowledge of IT systems, including the ability to use word processing skills, emails and the internet to create simple plans and reports. -Ability to work as a team member and autonomously. Additionally, the ability to work under pressure and to meet deadlines - Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face -Ability to organise, plan and prioritise on own initiative, including when under pressure and meeting deadlines.
