Job summary
We are looking for an Experienced Practice Nurse (Full or Part Time), ideally with at least 2-3 yrs experience in general practice to join our large, friendly GP practice. The right person will complement our forward-thinking nursing team working alongside GPs, Physician Assistants, Clinical Pharmacists, a Social Prescriber, MSK Physios and Drugs Counsellor.
You will become a member of a supportive and professional nursing team (5 Practice Nurses and an Assistant Practitioner), benefit from an experienced group and become a core member of the Practice in providing effective and patient centric care. The practice is involved with the local PCN & local INT giving you access to a wider team, training opportunities and a wider supportive network.

Main duties of the job
The practice nurse will work as part of the primary healthcare team providing Personal Medical Services to the patients of the practice; main responsibilities include:
Running independent LTC clinics e.g. Diabetes and/or Asthma
Cytology
Wound care, removal of sutures and venepuncture
Chronic disease management
Routine adult and childhood immunisations
SMI depot administration and management
Home visits as part of routine vaccination & covid booster programmes and LTC reviews
Work closely with practice MDT colleagues e.g. clinical pharmacists, physician assistants and care co-ordinators for example on diabetes care
The nursing duties cover extended hours which include Saturdays , shared fairly among the team from 9am 4pm

Job SummaryThe practice nurse will be expected to support patients with LTCs in clinics to provide a personalised service. Provide care in the treatment room for the patients of Paxton Green Group Practice. This would normally be carried out by face-to-face consultations including running LTC clinics independently, adult and childhood immunisations, dressings & core nursing services. The postholder will also be expected to work during extended hours & this will include Saturdays whole day on fair share.
Areas of Clinical Responsibility
To carry out nursing care plans and treatments in conjunction with MDT or independently to agreed protocols.
To work in accordance with clinical guidelines and practice policies
To carry out health screening as required
To carry out Long Term Conditions management
To carry out all general treatment room duties including, dressings etc.
To carry out adult & childhood immunisations
Undertake NHS health checks and Holistic Health Assessments, participate in research projects, as & when appropriate.
To maintain appropriate records and statistics
To communicate any abnormal findings to other members of the nursing team and/or the GPs, as appropriate
To visit patients at home as and when appropriate
To be active in health promotion and patient education
To carry out specialist nursing activities as appropriate and to participate in training and development
To work within the principles of the Practices PMS contract. and Quality & Outcomes Framework (QOF) and the nGMS contract
Attending primary health care team meetings and other meetings as may be necessary for your duties
Professional1. To adhere to the NMC code (Nursing & Midwifery Council) of Professional Conduct and to work in accordance with the scope of professional practice
2. To take responsibility for ones own professional practice and educational updates, in accordance with Prep and the needs of the practice
3. Participate in activities that lead to personal and/or team development

SupportWe normally arrange for all new staff members to have a thorough induction within the practice team.
All staff have regular appraisals and have access to appropriate training and development.
We have daily nurse meetings in addition to 6 weekly nurse meetings
Senior support in meetings once a week and monthly practice development forum (staff lead)
Time out for clinical supervision
