About this role

Job summary

The Advanced Clinical Practitioner (ACP) is an experienced autonomous clinician who works at an advanced level of practice, using expert clinical knowledge and skills to assess, diagnose, treat and manage patients presenting with undifferentiated and complex conditions.

Working within their scope of practice, the post holder will make independent clinical decisions, interpret investigations, initiate and evaluate treatment plans, and prescribe medication where appropriately qualified. The ACP will provide high-quality, patient-centred care, helping patients access timely assessment and treatment while reducing the need for GP appointments where appropriate.

The successful candidate will work collaboratively as part of the multidisciplinary team and will demonstrate the capabilities expected of advanced clinical practice across the four pillars of practice: clinical practice, leadership and management, education, and research. Applicants will hold a Master's degree in Advanced Clinical Practice (or equivalent) and be able to evidence the core capabilities and profession-specific competencies relevant to the role.

Main duties of the job

The Advanced Clinical Practitioner (ACP) will work as an autonomous clinician within the multidisciplinary primary care team, providing expert assessment, diagnosis, treatment and management of patients presenting with undifferentiated, acute and long-term conditions.

The post holder will independently assess patients, formulate management plans, request and interpret investigations, prescribe medication where appropriately qualified, and refer patients to secondary care or other services when required. They will provide high-quality, patient-centred care, helping to improve access and deliver timely clinical interventions.

The ACP will contribute to the delivery of chronic disease management, health promotion and preventative care, while supporting the practice to achieve quality improvement and clinical governance objectives. They will work collaboratively with GPs and the wider practice team, act as a clinical role model, contribute to education and service development, and support the delivery of safe, effective and evidence-based care in line with the four pillars of Advanced Clinical Practice.

Job description Job responsibilities

The Advanced Clinical Practitioner (ACP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.

The post holder works at the level of advanced clinical practice and has developed and can evidence the underpinning competencies applicable to the specialty or subject area, i.e., the knowledge, skills and behaviours appropriate to the healthcare professionals (HCP) setting and job role.

The role provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.

An ACP is autonomous in making decisions based on assessment, diagnosis and interpretation of test results and will initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.

Work to a masters level aware or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific competencies.

Primary key responsibilities

The following are the core responsibilities of the Advanced Clinical Practitioner.

Secondary responsibilities

In addition to the primary responsibilities, the Advanced Clinical Practitioner may be requested to:

Support the IPC lead, ensuring compliance, and undertaking audit as necessaryMonitor and ensure the safe storage, rotation and disposal of medicinesProviding guidance when necessary to colleagues, acting as a mentor to students and newly qualified staffParticipate in local initiatives to enhance service delivery and patient careSupport and participate in shared learning within the organisationDevelop an area of specialist interest, taking the lead within the organisationContinually review clinical practices, responding to national policies and initiatives where appropriateParticipate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)Commensurate with their role, support the development of clinical services within the organisation, liaising with external agencies and professional organisations as requiredDevelop practice administrative and clinical protocols in line with the needs of the patient and current legislation
