Job summary
32 hours per week.
To provide a dual-function role combining HR administrative and business support services with frontline health and wellbeing support for patients. The postholder will support staff and patients by promoting wellbeing, ensuring compliance with HR policies, and delivering a high-quality, person-centred service.
You will be expected to support the Primary Care Network Manager in overseeing the smooth running of all aspects of the Primary Care Network.
You will ensure new Staff details are recorded in the Personnel Databases and be the primary administrator for these.
The purpose of this role is to ensure that the PCN adheres to our policies and procedures for Health and Safety and Employment and that all of our staff are aware of their responsibilities.
You will be the key contact for all administration matters relating to staff in the Primary Care Network and you will provide an efficient, timely service for recruitment opportunities, the induction programme, training and the upkeep of personnel files.
Main duties of the job
The Administrator is responsible for the administrative process for recruitment and selection of Staff and for ensuring they are fully inducted. The post holder will also ensure that reviews are being undertaken and recorded.
Communication is key in this role; therefore, you will feel comfortable in setting up and hosting meetings, answering queries and being a key link between all the members of the PCN and all 5 practices. The flow of information is absolutely essential in this role.
Patient Facing Health and Wellbeing Support
Deliver frontline support to patients/clients, including: Initial wellbeing assessments Signposting to appropriate services (mental health, social care, community support) Provide guidance on: Lifestyle improvements (e.g. physical activity, stress management) Accessing healthcare or support services Build trusting relationships with patients/service users Support delivery of wellbeing programmes, workshops, or clinics Maintain accurate and confidential patient records Escalate safeguarding concerns where necessary

Undertake and complete an annual personal development plan (PDP) agreed between you and the PCN Board.
Participate in and support other staff in ensuring the PCN meets all relevant targets
Supporting the PCN and practice staff and respond to requests for advice and assistance.
Attend all relevant meetings.
Undertake additional training where necessary.
Awareness of and compliance with all relevant PCN and practice policies and guidelines.
Meet timescales and deadlines for audits and written returns to ensure that the PCN meets quality standards and receives the designated funding.
Person Specification
Qualifications Essential
Personalised Care Institute accredited health coach training (training can be provided to achieve this). Good standard of educations, GCSEs or equivalent particularly in English and Maths.
Experience Essential
Previous experience in an administration role. Proven ability in setting up systems and processes. Knowledge of PCNs and Primary Care practices and procedures. Understanding of working with Primary Care Staff. Experience in HR administration & employee relations. Administrative and business support. Patient-facing or customer-facing role. A highly proficient level of computer literacy particularly in Microsoft Word, Outlook, Web, PowerPoint, Excel, etc. The ability to motivate self and others. Commitment to reducing health inequalities and proactively working to reach people from diverse communities. Ability to actively listen, empathise with people and provide personalised support in a non-judgmental way. Effective organisational and time management skills, with the ability to prioritise and meet deadlines. Effective communication/listening skills at all levels. Ability to set up and maintain manual and computerised filing/HR systems. Ability to work proactively and to demonstrate good levels of initiative and self-reliance. Analytical and problem-solving skills, with the ability to manipulate complex statistical information effectively. Flexibility and adaptability. Attention to detail and accuracy. Honest and Trustworthy. Self-aware and possess cultural awareness and sensitivity. Driving Licence and access to own transport. May involve working across sites or in community settings. Some flexibility in working hours may be required.
Desirable
Working knowledge of TPP/Agilio TeamNet databases. Experience of working in a PCN/Primary Care setting. Understanding of equality, diversity and inclusion. Experience delivering wellbeing programmes. Ability to provide motivational coaching to support peoples behaviour change. Experience or training in personalised care and support planning.
