About the Role
We are seeking an experienced and motivated Paramedic Practitioner or Advanced Nurse Practitioner to join our Practice at Perranporth Surgery.
The post-holder will work autonomously within a supportive GP practice environment while being fully integrated into the wider multidisciplinary team. The role supports the ongoing work to improve access to care and reducing GP workload, while delivering high quality, patient centred care.
The role includes undertaking home visits, care homes, managing urgent and same day demand, and supporting patients with acute and long term conditions. The post-holder will also be responsible for managing their own clinical workload, including referrals and results.

Undertake advanced clinical assessment, diagnosis, treatment and management of patients in primary care
Support delivery of practice priorities and improved access in line with instructed requirements
Carry out planned and urgent home visits
Initiate, manage and follow up referrals, ensuring appropriate onward care
Review, action and communicate investigation results in a timely and clinically appropriate manner
Manage acute, urgent and complex clinical presentations autonomously
Work collaboratively with GPs, nurses, pharmacists and other ARRS funded roles
Maintain accurate, timely and high quality clinical records
Contribute to service development, transformation and continuous improvement

Perranporth Surgery is run as a GP Partnership made up of five motivated partners. We have an experienced multidisciplinary team including salaried GPs, Paramedics, Advanced Nurse Practitioner, Practice Nurses, and Health Care Assistants. We are also a training practice and regularly have GP Registrars, Foundation Doctors, and Medical Students in the surgery. We work closely within our Coastal PCN and have several ARRS roles working in house which include Mental Health Practitioners, First Contact Physiotherapists, Paramedics, Pharmacy Technicians, Care Co-ordinators, Health and Wellbeing Coaches and Dietitians. We also have an experienced administration team including receptionists, adminstrators, QOF administrators and a workflow team. Our knowledgeable Dispensers are led by a Dispensary Team Lead.

Registered Paramedic (HCPC) or Registered Nurse (NMC)
Competent in managing own referrals, investigations and clinical results
Willing and able to undertake home visits
Excellent communication and team working skills
Desirable Criteria
Experience working in primary care, community or PCN settings
Qualified Independent Prescriber
MSc level qualification or equivalent advanced clinical practice experience (or working towards where appropriate)
Experience working autonomously in an advanced clinical role
Knowledge of local services, referral pathways and community provision
