About this role

Job summary

Must be able to work between 07:45 - 18:30 and have own transport to travel between sites.

The Care Navigator is primarily responsible for undertaking a wide range of reception duties and the provision of general support to the wider multi-professional team. A sound professional working relationship will be required with the GPs and staff working within the Practice. We are expanding the reception team and the role requires high levels of organisation and excellent interpersonal skills.

The role includes:

Greeting and signposting patientsEffective use of the appointments system, booking appointments and processing informationActing as the central point of contact for patientsLiaising with multi-professional team members and external agencies such as secondary care Familiarity with Docman and EMIS Web clinical software systems

Main duties of the job

The post requires excellent communication skills, high levels of organisation and attention to detail as well as the ability to exercise tact, discretion and initiative. The ability to work under pressure and as part of a team are equally important.

The Practice will support continuing education and professional development in the form of attendance at study days, courses and with in-house training, as deemed appropriate for the benefit of both the individual and the Practice.

Job description Job responsibilities

Open up surgery, switching on clinicians PCs and ensuring everything is set up for the dayEnsure building security, have thorough knowledge of doors, windows and alarmsKeep reception and waiting rooms areas tidyBooking appointments face to face and over the phone and signposting to ensure the patient is booked with the right person, in the right place at the right timeChecking patients in for appointments and directing to appropriate waiting areaReceiving and recording requests for home visits, DN visits, telephone consultations and taking messagesGiving normal test results to patients or chasing up when necessaryAccess incoming emails via generic inbox , online consultations and web contacts signposting and taking all the appropriate actionsWork face-to-face as part of the CMP urgent care team currently based at Ethelbert Road - to ensure triage and administrative lists are dealt with appropriatelyCompleting patient registrations and adding details on to the computer when necessaryData input of new patient questionnairesSupporting GP Links processes such as preparing notes for sending off, filing incoming notes and ensuring filed medical records are accurately storedProcess patients change of address, numbers, etc. Process prescription requests in accordance with practice guidelines including filing prescriptions and recording in pharmacy bookFiling prescriptions and recording in pharmacy bookHanding completed repeat prescriptions to patients, checking name and addressReceiving and recording payments for medicals, insurance reports, etcGeneral administrative tasksMaking sure there is good supply of, and printing when necessary, practice leaflets, new patient questionnaires, change of address forms etcMonitoring the clinics and letting patients know when delays and clinicians when late visits, etc addedCompleting DN referrals and emailing to referral centreHelping to assist with general enquiriesAnswer incoming calls in a timely manner as per practice protocolCheck room availability for all clinicians each day and ensure check in screen shows any changesBe able to cover all care navigator roles as required including extended hour surgeriesHave knowledge of and work in accordance with practice policies and proceduresPulling/filing notes as necessaryScan and photocopy as requiredEnsure post and clinical forms are correctly filed and waiting for collectionEnsuring accurate recording of information using the practice clinical systemsEnsuring effective handover of matters outstanding to the incoming receptionists at end of shiftMake sure post tray is cleared every eveningComplete team EMIS & DOCMAN tasksContacting patients on the doctors behalf to arrange follow up appointments, prescription collections, test results etcOrdering ambulances as and when required by the clinicianChaperone for cliniciansGreeting visitors and asking them to sign the visitors book, informing appropriate managers/colleagues of their presence and, when necessary, show or direct them to the appropriate place ensuring passes/fobs are returned on leaving CMPAdding patient lists to the practice computer system for visiting consultants/therapists Passing on correct contact information when patients enquire about non CMP clinicsTaking deliveries or directing to relevant departmentAny other duties commensurate with the role

General Essential

Qualifications Essential

Good standard of education with good literacy and numeracy skills.

Desirable

Up to date with mandatory and role specific training.

Experience Essential

Good interpersonal and communication skills. Highly organised and excellent attention to detail. Time management and ability to prioritise workload. Knowledge of EMIS Web Clinical System and Docman. Ability to extract relevant information from conversations to pass on. Ability to work autonomously. Ability to follow policy and procedures. Accuracy in recording data.

Desirable

Good knowledge of Microsoft Office software.
