Job summary
Abbey Medical Group is seeking an experienced Practice Nurse to join our friendly and supportive team on a permanent basis and will ideally be available to work between 3037.5 hours per week.
Abbey Medical Group is a progressive training practice serving approximately 12,600 patients across two sites in Ravenshead and Blidworth, Nottinghamshire. The successful candidate will play a key role within our nursing team, contributing to both our core service provision and Extended Access clinics across both sites.

Main duties of the job
This forward-thinking GP practice is offering an excellent opportunity for an experienced Practice Nurse to join a dedicated clinical team committed to delivering outstanding patient care.
You will undertake a variety of nursing clinics and have the opportunity to further develop any specialist interests. Experience in the management of long-term conditions, particularly Diabetes and Respiratory Disease, would be highly desirable.
You will receive ongoing support and development opportunities whilst working within a team that places patients at the centre of everything it does.
In addition to standard Practice Nurse competencies, including Travel Immunisations, Childhood Immunisations and Cervical Screening, applicants must be NMC registered and ideally have a minimum of two years' experience within General Practice.
The post holder will be expected to work independently, exercising sound clinical judgement and demonstrating flexibility in relation to rotas and working patterns.

WE ARE ONLY CONSIDERING EXPERIENCED PRACTICE NURSES.
The following are the core responsibilities of the practice nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
Primary responsibilities
a. To develop, implement and embed health promotion and well-being programmes
b. To implement and evaluate individual treatment plans for chronic disease patients
c. To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health
d. To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
e. To provide wound care (ulcer/Doppler etc.) to patients
f. To provide travel medicine services
g. To request pathology services as necessary
h. To process pathology results as required
i. To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
j. To maintain accurate clinical records in conjunction with extant legislation
k. To ensure read codes are used effectively
l. To maintain chronic disease registers
m. To develop, implement and embed well woman clinics
n. To develop, implement and embed well man clinics
o. To chaperone patients where necessary
p. To prioritise health issues and intervene appropriately
q. To support the team in dealing with clinical emergencies
r. To recognise, assess and refer patients presenting with mental health needs
s. To implement vaccination programmes for adults and children
t. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
u. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
v. To support the clinical team with all safeguarding matters in accordance with local and national policies
w. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
x. To deliver opportunistic health promotion where appropriate
Secondary responsibilities
In addition to the primary responsibilities, the practice nurse may be requested to:
a. Support junior members of the nursing team, providing guidance when necessary
b. Participate in local initiatives to enhance service delivery and patient care
c. Support and participate in shared learning within the practice
d. Continually review clinical practices, responding to national policies and initiatives where appropriate
e. Participate in the review of significant and near-miss events applying a structured approach, i.e. root cause analysis (RCA)

Qualifications Essential
Registered nurse. Experience of working in a primary care environment. Experience of chronic disease management. Wound care/removal of sutures and staples. ECGs. Venepuncture. New patient medicals. Chaperone procedure. Requesting pathology tests and processing the results, advising patients accordingly. Travel medicine. Diabetes. Hypertension. Asthma. Spirometry. CHD. Immunisations (routine, childhood and travel). Womens health (cervical cytology, contraception, etc.). Understanding the importance of evidence based practice. Broad knowledge of clinical governance. Ability to record accurate clinical notes. Ability to work within own scope of practice and understand when to refer to GPs. Understanding of infection prevention and control measures. Knowledge of health promotion strategies. Understand the requirement for PGDs and associated policy. Excellent communication skills (written and oral). Strong IT skills. Clear, polite telephone manner. Competent in the use of Office and Outlook. Systmone user skills Effective time management (planning and organising). Ability to work as a team member and autonomously. Good interpersonal skills. Problem solving and analytical skills. Ability to follow clinical policy and procedure.
