Responsibilities include coordinating care services, matching carers with clients, supporting new care package setups, planning rotas for continuity, and communicating with families and healthcare professionals. Ideal candidates will be organised, people-focused, and able to confidently work in a fast-paced environment. A full UK driving licence, vehicle access, and at least 12 months remaining on your right to work in the UK are essential. Person Specification

Qualifications Essential
